宣透郁热、畅达玄府法治疗阻塞性睡眠呼吸暂停合并2型糖尿病探析OA
Exploration on Dispersing Stagnant Heat and Facilitating the Sweatpore Method in the Treatment of Obstructive Sleep Apnea Complicated with Type 2 Diabetes Mellitus
阻塞性睡眠呼吸暂停(OSA)合并2型糖尿病(T2DM)属于气血津液障碍,与"阳热怫郁-玄府闭塞-气液失运"密切相关.玄府为气血津液流通及神机运转的基本通路,因饮食、情志失调,内生诸邪,郁而化热,OSA合并T2DM患者处于"阳热怫郁"的病理状态,热郁气结则玄府郁闭,腠理闭塞,气液流通障碍,神机运转失利.针对OSA合并T2DM郁和热的病机特点,治疗应重视清、宣、透三法的运用,以宣透郁热、畅达玄府,使气液运转复常,神机得用,以解OSA合并T2DM"阳热怫郁"之态.
Obstructive sleep apnea(OSA)combined with type 2 diabetes mellitus(T2DM)belongs to disorders of qi,blood,and body fluids,which is closely related to the"excessive heat and congestion-obstruction of the sweatpore-failure of qi and fluid transportation"syndrome.The sweatpore is the basic pathway for the circulation of qi,blood,and body fluids as well as the operation of the spirit mechanism.Due to dietary and emotional imbalances,internal pathogenic factors accumulate and become hot,leading to OSA combined with T2DM in a pathological state of"excessive heat and congestion".When heat accumulates and blocks the qi,the sweatpore becomes obstructed,the pores close,and the circulation of qi and fluids is disrupted,resulting in the failure of the operation of spirit mechanism.In response to the pathogenic mechanism of congestion and heat in OSA combined with T2DM,treatment should focus on the application of the three methods of clearing,promoting,and penetrating,in order to disperse the depressed heat and facilitate the operation of the sweatpore,restore the normal circu-lation of qi and fluids,and enable the operation of the spirit mechanism,thereby alleviating the state of"excessive heat and congestion"in OSA combined with T2DM.
