SII、NLR、PLR、LMR对食管癌放疗患者近期预后的预测价值OA
Predictive value of SII,NLR,PLR,and LMR for short-term prognosis in esophageal cancer patients undergoing radiotherapy
目的 分析全身免疫炎症指数(SII)、中性粒细胞与淋巴细胞比值(NLR)、血小板与淋巴细胞比值(PLR)、淋巴细胞与单核细胞比值(LMR)对食管癌放疗患者近期预后的预测价值.方法 选取食管癌放疗患者82 例作为观察组,根据随访12 个月的结果,将其分为预后不良组(33 例)和预后良好组(49 例).另选取同期健康体检者 41 例作为对照组.比较观察组与对照组、预后不良组与预后良好组外周血SII、NLR、PLR、LMR值.采用ROC曲线分析各指标及其联合应用对近期预后的预测价值.结果 观察组外周血SII、NLR、PLR值均高于对照组,LMR值低于对照组(P<0.05).预后不良组外周血 SII、NLR、PLR值均高于预后良好组,LMR值低于预后良好组(P<0.05).ROC曲线分析结果显示,SII、NLR、PLR、LMR联合预测近期预后的AUC高于各指标单独检测值,其灵敏度为78.79%,特异度为85.71%.结论 食管癌放疗患者外周血SII、NLR、PLR、LMR值呈现异常表达,联合检测此四项指标有助于提高对患者近期预后的预测效能.
Aim To analyze the predictive value of systemic immune-inflammation index(SII),neutrophil-to-lymphocyte ratio(NLR),platelet-to-lymphocyte ratio(PLR),and lymphocyte-to-monocyte ratio(LMR)for the short-term prognosis of esophageal cancer patients undergoing radiotherapy.Methods A total of 82 esophageal cancer patients who received radiotherapy were enrolled as the observation group.Based on the 12-month follow-up outcomes,they were divided into a poor prognosis group(33 cases)and a good prognosis group(49 cases).Another 41 healthy individuals undergoing physical examination during the same period were selected as the control group.Peripheral blood SII,NLR,PLR,and LMR values were compared between the observation group and the control group,as well as between the poor and good prognosis groups.The ROC curve was used to analyze the predic-tive value of individual and combined indicators for short-term prognosis.Results The peripheral blood SII,NLR,and PLR val-ues in the observation group were higher than those in the control group,while the LMR value was lower(P<0.05).In the poor prognosis group,SII,NLR,and PLR values were higher than those in the good prognosis group,whereas the LMR value was lower(P<0.05).ROC curve analysis showed that the AUC for the combined prediction of short-term prognosis by SII,NLR,PLR,and LMR was higher than that of each indicator alone,with a sensitivity of 78.79%and a specificity of 85.71%,respectively.Con-clusion Peripheral blood SII,NLR,PLR,and LMR values are abnormally expressed in esophageal cancer patients undergoing radio-therapy.The combined detection of these indicators can improve the predictive efficacy for short-term prognosis.
