增强型体外反搏治疗慢性心力衰竭的疗效及对心脏超声参数的影响OA
Efficacy of enhanced external counterpulsation in the treatment of chronic heart failure and its effects on echocardiographic parameters
目的 探讨增强型体外反搏治疗慢性心力衰竭(CHF)的疗效及对超声心动图参数的影响.方法 选取 134 例CHF患者,随机均分为观察组(增强型体外反搏联合达格列净)和对照组(达格列净).比较两组治疗前、后超声心动图参数、血清心肌酶谱和N末端脑钠肽前体(NT-proBNP)水平、生活质量[明尼苏达心力衰竭生活质量问卷表(MLHFQ)]、6 min步行距离(6MWD)等情况.结果 与治疗前比较,两组治疗后左室射血分数升高,6MWD更长,左室舒张末内径、左室收缩末内径、血清NT-proBNP及MLHFQ评分均降低,且上述指标的改善幅度观察组均显著优于对照组(P<0.05).结论 增强型体外反搏的应用可有效提升临床疗效,抑制心室重塑,促进心脏结构重塑,改善心功能,提高生活质量.
Aim To investigate the efficacy of enhanced external counterpulsation(EECP)in the treatment of chronic heart failure(CHF)and its effects on echocardiographic parameters.Methods One hundred and thirty-four patients with CHF were selected and randomly divided into an observation group(treated with EECP combined with dapagliflozin)and a control group(treated with dapagliflozin).Echocardiographic parameters,serum myocardial enzyme profiles,N-terminal pro-brain natriuretic peptide(NT-proBNP),quality of life(assessed by the Minnesota Living with Heart Failure Questionnaire(MLHFQ)),and 6-minute walking dis-tance(6MWD)were compared between the two groups before and after treatment.Results Compared with the levels before treatment,after treatment,both groups showed increased left ventricular ejection fraction(LVEF)and longer 6MWD,as well as de-creased left ventricular end-diastolic diameter(LVEDD),left ventricular end-systolic diameter(LVESD),serum NT-proBNP levels,and MLHFQ scores.Furthermore,the improvement in all these indicators was significantly greater in the observation group than that in the control group(P<0.05).Conclusion The application of EECP can effectively improve clinical efficacy,in-hibit ventricular remodeling,promote cardiac structural remodeling,enhance cardiac function,and improve quality of life in patients with CHF.
