目的 探讨白蛋白Super-ISH在肝细胞癌和肝内胆管癌病理诊断及鉴别诊断中的应用价值.方法 选取207例肿瘤组织(包括53例肝细胞癌、28例肝内胆管癌、9例混合性肝细胞癌-胆管癌以及117例其他多种器官恶性上皮性肿瘤)和6例正常肝组织石蜡标本,使用Super-ISH检测上述组织中白蛋白mRNA表达情况,并进行分析.结果 52例(52/53,98.1%)肝细胞癌白蛋白mRNA呈阳性,其中48例(48/53,90.6%)显示弥漫强阳性;白蛋白mRNA的表达在原发灶或转移性、不同分化程度、不同组织块大小以及不同储存年限石蜡组织的肝细胞癌中差异无统计学意义(P均>0.05).免疫组化结果显示:肝细胞癌中肝细胞抗原(hepatocyte paraffin 1,Hep-Par-1)、精氨酸酶1(arginase-1,Arg-1)及磷脂酰肌醇蛋白聚糖3(glypican-3,GPC-3)的阳性率分别为79.2%(42/53)、84.9%(45/53)及69.8%(37/53),在低分化原发灶肝细胞癌中,三者阳性率分别为68.4%(13/19)、78.9%(15/19)及73.7%(14/19),三者在诊断肝细胞癌的敏感性均低于白蛋白mRNA.6例肝内胆管癌(6/28,21.4%)白蛋白mRNA呈异质性表达,且均为小胆管亚型,大胆管亚型未见表达;肝脏混合型肝细胞癌-胆管癌白蛋白mRNA阳性率为88.9%(8/9),其中肝细胞癌成分呈弥漫阳性,胆管癌成分呈异质性表达.7例肝样腺癌(7/19,36.8%)中白蛋白mRNA弥漫或局灶阳性;其他18种不同类型多种器官恶性上皮性肿瘤中,分别有1例胰腺腺泡细胞癌和卵黄囊瘤呈白蛋白mRNA局灶阳性.本组病例中,白蛋白mRNA诊断肝细胞癌的敏感性为98.1%(52/53),特异性为85.1%(131/154);诊断肝内胆管癌的敏感性为21.4%(6/28),特异性为61.5%(110/179).结论 白蛋白Super-ISH法对于识别肝细胞癌具有高度敏感性和特异性,对不同分化程度、不同组织块大小以及不同储存年限的肝细胞癌均具有重要的诊断及鉴别诊断价值;白蛋白mRNA有助于区分肝内胆管癌亚型,但并不是肝内胆管癌的高灵敏性标志物.

Objective To investigate the diagnostic value of albumin Super-ISH in the pathological diagnosis and differential diagnosis of hepatocellular carcinoma(HCC)and intrahepatic cholangiocarcinoma(ICC).Methods A total of 207 tumor specimens,including 53 HCCs,28 ICCs,9 combined hepatocellular-cholangiocarcinomas(cHCC-CCs),and 117 malignant epithelial tumors from various organs,as well as 6 normal liver tissue samples,were ana-lyzed.Albumin mRNA expression was detected using Super-ISH and systematically evaluated.Results Among HCCs,52 of 53 cases(98.1%)were positive for albumin mRNA,with strong diffuse staining observed in 90.6%(48/53).No significant differences were observed in albumin mRNA expression with respect to primary versus metastatic lesions,degree of differentiation,tissue block size,or storage duration of paraffin-embedded tissues(all P>0.05).Immunohistochemically,the positive rates of hepatocyte paraffin 1(HepPar-1),arginase-1(Arg-1),and glypican-3(GPC-3)in HCCs were 79.2%(42/53),84.9%(45/53),and 69.8%(37/53),respectively;in poorly differentiated primary HCCs,the corresponding rates were 68.4%(13/19),78.9%(15/19),and 73.7%(14/19).The sensitivi-ties of these three markers were all lower than that of albumin mRNA.Albumin mRNA expression was detected in 6 of 28 ICCs(21.4%),all of which were small bile duct subtypes,whereas no expression was observed in large bile duct subtypes.Among cHCC-CCs,8 of 9 cases(88.9%)were positive,showing diffuse expression in the HCC component and heterogeneous expression in the cholangiocarcinoma component.Albumin mRNA was diffusely or focally positive in 7 of 19 hepatoid adenocarcinomas(36.8%).Among the other 18 different types of malignant epithelial tumors of multiple organs,only one pancreatic acinar cell carcinoma and one yolk sac tumor exhibited focal albumin mRNA ex-pression.In this cohort,the sensitivity and specificity of albumin mRNA for diagnosing HCC were 98.1%(52/53)and 85.1%(131/154),respectively,while those for diagnosing ICC were 21.4%(6/28)and 61.5%(110/179).Conclusion Albumin Super-ISH demonstrated high sensitivity and specificity for identifying HCC and showed stable performance across tumors of different differentiation grades,tissue sizes,and storage durations,supporting its signifi-cant diagnostic and differential diagnostic value.Albumin mRNA expression was helpful in distinguishing ICC sub-types but was not a highly sensitive marker for ICC.
