目的:探讨基于积极心理学理论的机器人辅助步态训练在下肢烧伤整形患者中的应用效果.方法:选取2020年7月—2024 年 7 月空军军医大学唐都医院收治的 96 例下肢烧伤整形患者为研究对象,采用随机数字表法将其分为对照组(n=48,予以常规康复训练)与观察组(n=48,予以基于积极心理学理论的机器人辅助步态训练).比较两组患者的心理弹性、瘢痕情况、下肢功能及不良事件发生情况.结果:与干预前相比,两组患者干预后心理弹性量表(CD-RISC)评分均升高,焦虑自评量表(SAS)、抑郁自评量表(SDS)评分均降低(P<0.05);观察组干预后CD-RISC评分较对照组更高,SAS、SDS评分较对照组更低(P<0.05).干预后,两组患者温哥华瘢痕量表(VSS)和瘢痕美容评估与评级量表(SCAR)评分均降低(P<0.05);观察组干预后VSS和SCAR评分均低于对照组(P<0.05).干预后,对照组R0显著降低,观察组R9显著降低,且观察组R0显著高于对照组,R9显著低于对照组(P<0.05).干预后,两组患者FMA、BBS评分均升高(P<0.05),且观察组FMA、BBS评分均较对照组更高(P<0.05).观察组不良事件发生率低于对照组(P<0.05).结论:基于积极心理学理论的机器人辅助步态训练可显著提高下肢烧伤整形患者的心理弹性,改善瘢痕及生活质量,具有潜在的临床应用价值.

Objective:To evaluate the clinical efficacy of robot-assisted gait training based on positive psychology theory in patients undergoing lower limb burn reconstruction.Methods:96 patients who underwent lower limb burn reconstruction at Tangdu Hospital from July 2020 to July 2024 were enrolled.They were randomly divided into the control group(n=48,receiving conventional rehabilitation training)and the observation group(n=48,receiving robot-assisted gait training based on positive psychology theory)using a random number table.Results:Compared with that before intervention,the Connor-Davidson resilience scale(CD-RISC)scores of the two groups were increased,while the self-rating anxiety scale(SAS)and self-rating depression scale(SDS)scores were decreased(P<0.05).Higher CD-RISC score and lower SAS and SDS scores were found in the observation group compared with the control group(P<0.05).Compared with that before intervention,both groups showed significant reductions in Vancouver scar scale(VSS)and scar cosmesis assessment and rating(SCAR)scores(P<0.05).Meanwhile,the VSS and SCAR scores of the observation group were all lower than those of the control group(P<0.05).After intervention,the R0 in the control group and the R9 in the observation group decreased significantly,and the observation group had significantly higher R0 and lower R9 scores than the control group(P<0.05).Significant improvements in Fugl-Meyer assessment(FMA)and Berg balance scale(BBS)scores were found in the two groups after intervention(P<0.05),with significantly higher FMA and BBS scores in the observation group than the control group(P<0.05).The incidence of adverse events was significantly lower in the observation group than that in the control group(P<0.05).Conclusion:Robot-assisted gait training based on positive psychology theory can significantly enhance psychological resilience,improve scar outcomes and quality of life,and promote functional recovery in patients underwent lower limb burn reconstruction,it has certain potential clinical application value.
