虚拟现实步行训练在帕金森病步态障碍患者中的临床应用OA

目的:探讨分析C-Mill虚拟现实行走训练智能跑台在帕金森病步态障碍患者中的临床应用效果.方法:选取 2024 年2 月—2025 年 5 月于首都医科大学附属北京康复医院接受康复治疗的 76 例帕金森病步态障碍患者,通过计算机生成随机数字表将患者分为C-Mill训练组(n=38,采用C-Mill虚拟现实行走训练智能跑台训练)和传统训练组(n=38,采用常规步态平衡训练).比较两组患者的步态时空参数(步速、步幅、步宽、步长变异性)、平衡功能[Berg平衡量表(BBS)评分]、运动功能[10米步行测试(10MWT)、计时起立-行走测试(TUGT)]、不良事件发生情况及康复训练满意度之间的差异.结果:C-Mill训练组在康复训练 3 周后的步速、步幅、BBS评分均优于传统训练组,步宽、步长变异性均小于传统训练组,10MWT及TUGT用时短于传统训练组,差异均有统计学意义(P<0.05).C-Mill训练组的不良事件总发生率低于传统训练组(18.42%vs.42.11%),总满意率高于传统训练组(86.84%vs.71.05%),差异有统计学意义(P<0.05).结论:C-Mill虚拟现实行走训练智能跑台能够有效改善帕金森病步态障碍患者的步态时空参数、平衡功能和运动功能,减少不良事件发生,提高患者对康复训练的满意度,值得临床推广应用.

Objective:To explore and analyze the clinical application effect of the C-Mill virtual reality(VR)walking training treadmill in Parkinson's disease(PD)patients with gait disorders.Methods:From February 2024 to May 2025,76 patients with gait disorders caused by Parkinson's disease who received rehabilitation intervention at Beijing Rehabilitation Hospital,Capital Medical University were randomly divided into the C-Mill training group(n=38,treated with C-Mill VR walking training treadmill)and the conventional training group(n=38,treated with conventional gait and balance training)using the computer-generated random number table.Differences in gait spatiotemporal parameters(gait speed,stride length,stride width,stride length variability),balance function[Berg balance scale(BBS)score],motor function[10-meter walk test(10MWT),timed up and go test(TUGT)],incidence of adverse events,and rehabilitation training satisfaction were compared between the two groups.Results:After 3 weeks of rehabilitation training,the C-Mill training group showed higher walking speed,stride length,and BBS score than the conventional training group,while stride width and stride length variability were significantly lower,and 10MWT and TUGT times were shorter,with statistically significant differences(P<0.05).The total incidence of adverse events was significantly lower in the C-Mill training group(18.42%vs.42.11%),and the overall satisfaction rate was significantly higher(86.84%vs.71.05%)compared to the conventional training group(P<0.05).Conclusion:The C-Mill VR walking training treadmill can effectively improve spatiotemporal gait parameters,balance function,and motor function in PD patients with gait disorders,reduce adverse event incidence,and enhance patient satisfaction with rehabilitation training,which is worthy of clinical application.
