目的:探究基于"6S"[整理(seiri)、整顿(seiso)、清扫(shitsuke)、清洁(seiton)、素养(seiketsu)、安全(security)]基础框架构建的目视管理模式在达芬奇Xi手术系统消毒管理中的应用效果.方法:分别将 2024 年 1 月—2024 年8 月和 2024 年 9 月—2025 年 5 月空军军医大学第一附属医院消毒供应中心(CSSD)收入的达芬奇Xi手术系统器械作为常规组(n=148,采取常规消毒模式)和研究组(n=151,采取基于"6S"基础框架构建的目视管理模式).对比两组器械清洗合格率、器械管理不良事件发生率及手术室器械使用人员满意度.结果:目测评估结果显示,两组器械的清洗合格率无显著差异(P>0.05);光源放大镜检测结果显示,研究组器械清洗合格率高于常规组(P<0.05).分别对目测评估和光源放大镜检测合格的器械进行ATP生物荧光检测,结果显示,研究组器械清洗合格率高于常规组(P<0.05).研究组器械遗失、器械包装或装载错误及超期限使用等不良管理事件发生率低于常规组(P<0.05),但两组器械损坏率比较,差异无统计学意义(P>0.05).研究组手术室器械使用人员对清洗后的机械臂灵活度、器械预处理方式、器械清洗质量、CSSD人员工作态度及工作效率的评分均高于常规组(P<0.05).结论:基于"6S"基础框架构建的目视管理模式能在一定程度提高达芬奇Xi手术系统的器械清洗合格率,降低器械管理不良事件的发生率,提高器械使用人员满意度.

Objective:To investigate the application effect of a visual management model constructed based on the"6S"framework(seiri,seiso,shitsuke,seiton,seiketsu,and security)in the disinfection management of Da Vinci Xi Surgical System.Methods:Da Vinci Xi surgical instruments sterilized at the Central Sterile Supply Department(CSSD)of the First Affiliated Hospital of Air Force Medical University were enrolled.Instruments sterilized from January 2024 to August 2024 were served as the conventional group(n=148,managed by conventional sterilization model)and instruments sterilized from September 2024 to May 2025 were divided into the study group(n=151,managed by a visual management model based on the"6S"framework),respectively.Instrument cleaning qualification rate,incidence of adverse management events,and satisfaction level of operating room personnel were compared between the two groups.Results:Under visual inspection,no statistically significant difference in cleaning qualification rate was observed between the two groups(P>0.05).However,under magnified light inspection,the study group showed a significantly higher cleaning qualification rate than the control group(P<0.05).Further ATP bioluminescence testing of instruments deemed qualified by both visual and magnified light inspections confirmed a significantly higher qualification rate in the study group than the conventional group(P<0.05).The study group showed significantly lower incidence rates of adverse events including instrument loss,incorrect packaging or loading,and use beyond expiration date(P<0.05),while no significant difference was found in instrument damage rate(P>0.05).The study group scored significantly higher than the conventional group in terms of user satisfaction including mechanical arm flexibility after cleaning,CSSD pre-processing method,CSSD staff attitude,instrument cleaning quality,and CSSD operational efficiency(P<0.05).Conclusion:The visual management model based on the"6S"framework can effectively enhance the cleaning qualification rate of Da Vinci Xi surgical instruments,reduce the incidence of adverse management events,and improve satisfaction level of operating room personnel.
