目的:探讨骨盆骨折患者在机器人辅助经皮通道螺钉固定术后伤口感染的危险因素并构建列线图预测模型.方法:选取 2020 年 10 月—2023 年 3 月于空军军医大学第一附属医院接受机器人辅助经皮通道螺钉固定术的 97 例患者为研究对象,根据术后是否发生感染分为感染组(n=41)和未感染组(n=56).采用多因素Logistic回归分析术后感染的危险因素并建立预测模型,评估该模型的预测准确性及临床实用性.结果:在感染组患者中共分离出41株病原菌,主要为金黄色葡萄球菌、大肠埃希菌、表皮葡萄球菌和铜绿假单胞菌.多因素分析显示,高BMI、高损伤严重度评分(ISS)、合并脑外伤、髋臼骨折及腹部损伤为骨盆骨折患者术后发生感染的危险因素(P<0.05).基于上述危险因素构建预测模型并绘制受试者工作特征曲线,曲线下面积为0.976(95%CI:0.945~1.000),且校准曲线接近理想曲线,提示该模型具有较好的区分能力和临床效能.进一步进行决策曲线分析,结果显示当模型预测的风险阈值>0.05 时,可带来临床净收益.结论:高BMI、高ISS评分、合并脑外伤、髋臼骨折及腹部损伤均为骨盆骨折患者术后发生感染的危险因素,基于以上因素构建的列线图模型能为临床早期评估骨盆骨折术后感染风险提供直观的可视化工具.

Objective:To investigate the risk factors for surgical site infection after robot-assisted percutaneous screw fixation in patients with pelvic fractures and to construct a nomogram prediction model.Methods:97 patients who underwent robot-assisted percutaneous screw fixation at the First Affiliated Hospital of Air Force Medical University from October 2020 to March 2023 were enrolled.They were divided into the infection group(n=41)and the non-infection group(n=56)based on the occurrence of postoperative infection.Multivariate Logistic regression analysis was used to identify risk factors for postoperative infection and to construct a predictive model.The predictive accuracy and clinical utility of the model were evaluated.Results:A total of 41 strains of pathogens were isolated from patients in the infection group,primarily Staphylococcus aureus,Escherichia coli,Staphylococcus epidermidis,and Pseudomonas aeruginosa.Multivariate analysis showed that BMI,injury severity score(ISS),concomitant traumatic brain injury,acetabular fracture,and abdominal injury were risk factors for postoperative infection in patients with pelvic fractures(P<0.05).A predictive model was constructed based on these risk factors,and the area under the receiver operating characteristic curve was 0.976(95%CI:0.945-1.000).The calibration curve was close to the ideal curve,indicating good discriminative ability and clinical efficacy of the model.Further decision curve analysis showed that when the risk threshold predicted by the model was>0.05,it could provide a net clinical benefit.Conclusion:BMI,ISS score,concomitant traumatic brain injury,acetabular fracture,and abdominal injury are risk factors for postoperative infection in patients with pelvic fractures.The nomogram model constructed based on these factors can provide an intuitive visualization tool for early clinical assessment of infection risk after pelvic fracture surgery.
