中医药调控炎症因子治疗放射性肠炎研究进展OA
Research Progress on Traditional Chinese Medicine in Regulating Inflammatory Factors for the Treatment of Radiation Enteritis
放射性肠炎(radiation enteritis,RE)是盆腔、腹腔、腹膜后恶性肿瘤患者经放射治疗后所引起的一种严重的肠道并发症,促炎/抗炎因子失衡及炎症介质级联反应是放射性肠炎发病的核心环节,本文系统梳理近年来中医药调控炎症因子干预RE的研究进展,发现中药单体及复方可通过调控炎症因子网络及相关信号通路[如核因子-κB(nuclear factor kappa-B,NF-κB)、核苷酸结合结构域富含亮氨酸重复序列和含热蛋白结构域受体3(nucleotide-binding domain leucine-rich repeat and pyrin do-main-containing receptor 3,NLRP3)、丝裂原活化蛋白激酶(mitogen-activated protein kinase,MAPK)等]发挥减轻肠道炎症、修复屏障功能、抑制细胞凋亡等多重作用,体现出多靶点、整体调节的独特优势,具有良好的临床应用潜力.但目前的研究存在以下问题:(1)药效机制解析不够精准,药物的直接作用靶点(如受体、激酶)、信号转导的具体过程及复方"君臣佐使"的协同机制研究不够深入;(2)研究多聚焦于单一通路,未能系统揭示中医药在多通路协同或拮抗中的整体调控作用;(3)研究以传统方药的验证为主,新复方、新配伍及肠道适宜新剂型研究有限.未来研究可结合分子对接、基因编辑、蛋白质组学等技术精准鉴定直接作用靶点,阐明复方成分的协同机制,构建系统调控网络;重点探讨研究中医药调控多条炎症通路协同模式;依托类器官、器官芯片等新型模型加强中药新药、新剂型及递送系统的创新研发与转化.
Radiation enteritis(RE)is a severe intestinal complication resulting from radiotherapy in patients with pelvic,abdominal,or retroperitoneal malignancies.The imbalance between pro-inflammatory and anti-inflammatory factors,along with the cascade of inflam-matory mediators,constitutes the core pathological link in the onset of RE.This paper has systematically reviewed recent research pro-gress on the intervention of RE by TCM through the regulation of inflammatory factors.It has revealed that TCM monomers and com-pound formulas can mitigate intestinal inflammation,repair barrier function,and inhibit cell apoptosis through multiple effects,such as regulating the inflammatory factor network and related signaling pathways(e.g.,NF-κB,NLRP3,MAPK).This demonstrates the u-nique advantages of multi-target action and holistic regulation,indicating promising potential for clinical application.However,this field still faces the following challenges:(1)The analysis of pharmacodynamic mechanisms is not sufficiently precise,with inadequate in-depth research on the direct targets of drug action(e.g.,receptors,kinases),the specific processes of signal transduction,and the syn-ergistic mechanisms underlying the"sovereign-minister-assistant-courier"principle within compound formulas;(2)Research predomi-nantly focuses on single pathways,failing to systematically elucidate the holistic regulatory role of TCM in the synergy or antagonism a-mong multiple pathways;(3)Research is primarily centered on the verification of traditional formulas,with limited research on new compound formulas,new compatibilities,and innovative intestinal-specific dosage forms.Therefore,future research could employ tech-nologies such as molecular docking,gene editing,and proteomics,etc.to precisely identify direct action targets and elucidate the syner-gistic mechanisms of compound formula components.Furthermore,integrating multi-omics approaches with network pharmacology meth-ods to construct systematic regulatory networks is crucial,with a focus on exploring the synergistic patterns of TCM in regulating multiple inflammatory pathways.Leveraging novel models like organoids and organs-on-chips,etc.will be key to strengthening the innovative de-velopment and translational research of new Chinese medicinals,novel dosage forms,and new drug delivery systems.
