大脑学习训练法对Lenke 1型青少年特发性脊柱侧凸的临床效果评估OA
Clinical evaluation of the Brain Learning Training Method for Lenke type 1 adolescent idiopathic scoliosis
目的:探讨大脑学习训练法(Brain Learning Training Method,BLTM)在Lenke 1型青少年特发性脊柱侧凸(adolescent idiopathic scoliosis,AIS)非手术治疗中的应用,评估BLTM对AIS患者脊柱结构、体态美学与生活质量的综合干预效果.方法:前瞻性纳入2025年2~8月接受BLTM干预的50例Lenke 1型AIS患者,训练内容包括脊柱延伸控制、呼吸模式重建与多通道感知反馈,通过对比BLTM干预前及末次随访的AIS患者的脊柱影像学参数、体态美学指标及生活质量评分,综合评价干预效果.结果:BLTM干预后,AIS 患者主胸弯 Cobb 角由 29.14°±9.28°减小至 22.69°±9.23°(P<0.01),腰椎前凸角(LL)由45.60°±8.60°减小至 39.33°±8.16°(P<0.01).骨盆入射角-腰椎前凸角(PI-LL)由 0.58°±9.31°增加至7.62°±6.04°(P<0.01),矢状面垂直轴距离(SVA)由(0.15±2.97)cm增加至(3.11±2.76)cm(P<0.01).体态美学方面,躯干旋转角(ATR)由5.45°±3.15°降至2.88°±2.41°(P<0.01),肩面积指数2(SAI2)中位数由98.62%降至92.54%(P=0.049).生活质量评估显示,VAS疼痛评分由(2.90±2.07)分降至(1.49±1.15)分(P<0.01),SRS-22量表总分均数由(3.74±0.47)分升高至(3.97±0.39)分(P<0.01).结论:BLTM训练在短期内改善了AIS患者的冠状畸形和躯干旋转,并在体态认知、自我形象及心理状态方面带来积极变化,但训练过程中矢状面平衡出现前移趋势,肩面积指标百分比较前降低,提示外观形态与视觉对称的恢复进程并不完全一致.该方法在AIS非手术治疗中具有应用价值,但其多维度的综合效应需在临床决策中予以充分考虑.
Objective:To investigate the application of Brain Learning Training Method(BLTM)in the non-surgical treat-ment of Lenke type 1 adolescent idiopathic scoliosis(AIS),and to evaluate the comprehensive effects of BLTM on spinal struc-ture,posture aesthetics,and quality of life in patients with AIS.Methods:A prospective study enrolled 50 patients with Lenke type 1 AIS who underwent BLTM intervention from February to August 2025.The training content included spine extension con-trol,respiratory pattern reconstruction,and multi-channel sensory feedback.By comparing the spinal imaging parameters,posture aesthetic indicators,and quality of life scores of AIS patients before and at the last follow-up,the intervention effect was compre-hensively evaluated.Results:After BLTM intervention,the Cobb angle of the main thoracic curve decreased from 29.14°±9.28° to 22.69°±9.23°(P<0.01),and the lumbar lordosis angle(LL)decreased from 45.60°±8.60°to 39.33°±8.16°(P<0.01).The pelvic inciclence-lunbar lordosis(PI-LL)increased from 0.58°±9.31°to 7.62°±6.04°(P<0.01),and the sagittal vertical axis(SVA)increased from(0.15±2.97)cm to(3.11±2.76)cm(P<0.01).In terms of body aesthetics,trunk rotation angle(ATR)decreased from 5.45°±3.15°to 2.88°±2.41°(P<0.01),and median shoulder area index 2(SAI2)decreased from 98.62%to 92.54%(P=0.049).Quality of life assessment showed that the VAS pain score decreased from 2.90±2.07 to 1.49±1.15(P<0.01),and the total score of SRS-22 increased from 3.74±0.47 to 3.97±0.39(P<0.01).Conclusion:BLTM training can improve coronal deformity and trunk rotation in AIS patients in the short term,and enhance body perception,self-image,and psychological well-being.However,sagittal balance showed a trend of forward movement and decreased shoulder area index percentage during the training,suggesting that the recovery of external morphology and visual symmetry may not occur synchronously.This method has application value in the non-surgical management of AIS,but its multidimensional comprehensive effects should be carefully considered in clinical decision-making.
