青年与老年男性肛周脓肿病原菌分布及药敏差异分析OA

Analysis of pathogen distribution and antibiotic susceptibility differences in perianal abscess between young and elderly males

目的 了解青年男性肛周脓肿患者脓液病原菌分布特点以及药物敏感性,指导临床合理使用抗菌药物.方法 收集2023年2月至2025年2月联勤保障部队第九〇八医院肛肠科收治、行肛周脓肿手术治疗的189例患者的原发性成脓期脓液标本进行培养和药敏分析,根据年龄分为青年男性组(128例)和老年男性组(61例).比较两组细菌培养和药敏试验结果.结果 青年男性组肛周脓肿脓液标本检出单一兼性厌氧菌84例(65.63%)、单一专性厌氧菌6例(4.69%)、混合感染11例(8.59%)、未培养出细菌27例(21.09%),老年男性组肛周脓肿脓液标本检出单一兼性厌氧菌51例(83.60%)、单一专性厌氧菌3例(4.92%)、混合感染4例(6.56%)、未培养出细菌3例(4.92%),两组细菌检出率比较,差异有统计学意义(P<0.05).两组检出大肠埃希菌占比分别为35.94%(46/128)、52.46%(32/61),肺炎克雷伯菌14.84%(19/128)、14.75%(9/61),金黄色葡萄球菌7.81%(10/128)、4.92%(3/61),单一专性厌氧菌均为脆弱拟杆菌.两组检出超广谱β内酰胺酶大肠埃希菌占比分别为26.09%(12/46)和53.13%(17/32),差异有统计学意义(P<0.05).药敏试验显示,青年男性组革兰氏阴性杆菌对头孢西丁的敏感率较高,为98.51%(66/67),对厄他培南、亚胺培南的敏感率分别为98.57%(69/70)和94.29%(66/70),对头孢菌素类头孢呋辛、头孢他啶和头孢吡肟的敏感率分别是67.65%(46/68)、77.14%(54/70)和80.00%(56/70),两组金黄色葡萄球菌对达托霉素、万古霉素、利奈唑胺和莫西沙星的敏感率均为100.00%,青年男性组革兰氏阳性球菌对红霉素的敏感率为64.29%(9/14).结论 青年男性肛周脓肿病原菌可优先选用兼顾专性及兼性厌氧菌的头霉素类抗菌药物(如头孢西丁)进行抗菌治疗,青年男性和老年男性肛周脓肿超广谱β内酰胺酶大肠埃希菌检出率存在差异,老年男性组更易检出.应根据肛周脓肿病原菌特点和药敏试验结果,及时调整使用有效的抗菌药物.

Objective To investigate the distribution characteristics of pathogenic bacteria in the purulent fluid of young male patients with perianal abscess and their antibiotic sensitivities,thereby guiding the rational use of antimicrobial agents in clinical practice.Methods Pus specimens from 189 hospitalized patients with primary suppurative perianal abscess who underwent surgical treatment in the Department of Proctology at 908th Hospital of the Joint Logistics Support Force from February 2023 to February 2025 were collected for culture and antibiotic susceptibility testing.These samples underwent culture and antimicrobial susceptibility analysis.Patients were divided into a young male group(n=128)and an elderly male group(n=61)based on age.Bacterial culture and susceptibility test results were compared between the two groups.Results In the young male group,among the pus specimens of perianal abscess,84 cases(65.63%)were detected with a single facultative anaerobe,6 cases(4.69%)with a single obligate anaerobe,11 cases(8.59%)with mixed infection,and 27 cases(21.09%)with no bacteria cultured.In the elderly male group,among the perianal pus specimens,51 cases(83.60%)were detected with a single facultative anaerobe,3 cases(4.92%)with a single obligate anaerobe,4 cases(6.56%)with mixed infection,and 3 cases(4.92%)with no bacteria cultured.The comparison of bacterial detection rates between the two groups showed a statistically significant difference(P<0.05).The detection rates of Escherichia coli in the two groups were 35.94%(46/128)and 52.46%(32/61)respectively;the rates of Klebsiella pneumoniae were 14.84%(19/128 cases)and 14.75%(9/61)respectively;the rates of Staphylococcus aureus were 7.81%(10/128)and 4.92%(3/61)respectively.All the detected single obligate anaerobes were Bacteroides fragilis.The detection rates of extended-spectrum β-lactamase-producing Escherichia coli(ESBL-producing E.coli)in the two groups were 26.09%(12/46)and 53.13%(17/32)respectively,with a statistically significant difference(P<0.05).Antimicrobial susceptibility test results showed that in the young male group,the Gram-negative bacilli had a relatively high susceptibility rate of 98.51%(66/67)to cefoxitin,the susceptibility rates to ertapenem and imipenem were 98.57%(69/70)and 94.29%(66/70)respectively,and the susceptibility rates to cephalosporins including cefuroxime,ceftazidime and cefepime were 67.65%(46/68),77.14%(54/70)and 80.00%(56/70)respectively.The Staphylococcus aureus strains in both groups showed a 100.00%susceptibility rate to daptomycin,vancomycin,linezolid and moxifloxacin.In the young male group,the Gram-positive cocci had a susceptibility rate of 64.29%(9/14)to erythromycin.Conclusions For perianal abscess in young males,antimicrobial therapy may prioritize cephamycins such as cefoxitin,which covers both obligate and facultative anaerobes.A significant difference exists in the detection rate of ESBL-producing E.coli between young and elderly males,with a higher rate observed in the elderly group.Based on the characteristics and susceptibility profiles of the pathogens,timely adjustment to effective antimicrobial agents is recommended.
