前梯度蛋白2和sTREM-1水平与慢性阻塞性肺疾病急性加重期肺部感染患者预后的关系OA
Relationship between levels of anterior gradient 2 and sTREM-1 and prognosis in patients with acute exacerbation of chronic obstructive pulmonary disease and pulmonary infection
目的 探讨前梯度蛋白2(AGR2)和可溶性髓系细胞触发受体1(sTREM-1)水平与慢性阻塞性肺疾病急性加重期(AECOPD)合并肺部感染患者预后的关系.方法 前瞻性选取2022年10月至2025年1月于郑州大学附属郑州中心医院收治的AECOPD合并肺部感染患者168例作为感染组,另选取基线资料匹配的AECOPD无肺部感染患者122例作为非感染组.比较两组一般资料和血清AGR2和sTREM-1水平,并比较不同肺部感染程度AECOPD患者血清AGR2和sTREM-1水平,分析AGR2和sTREM-1水平对AECOPD患者肺部感染的预测价值,比较两组预后情况,并比较不同预后患者AGR2和sTREM-1水平.结果 感染组AGR2和sTREM-1水平[分别为(25.31±4.40)ng/mL和(168.19±21.06)pg/mL]显著高于非感染组[分别为(16.49±3.43)ng/mL和(149.42±16.83)pg/mL,t值分别为19.162、8.429,P<0.001].轻度感染、中度感染、重度感染患者AGR2和sTREM-1水平依次升高(P<0.001).受试者操作特征曲线分析显示,AGR2和sTREM-1联合检测AECOPD患者肺部感染的曲线下面积为0.951.感染组预后良好率为59.39%(98/165),显著低于非感染组的80.17%(97/121,P<0.001).预后不良患者AGR2[(27.00±4.03)ng/mL]和sTREM-1水平[(173.26±20.51)pg/mL]显著高于预后良好组[分别为(24.53±4.28)ng/mL和(163.82±21.65)pg/mL,t值分别为3.727、2.810,P<0.01].结论 AECOPD合并肺部感染患者血清AGR2和sTREM-1水平呈高表达,且预后相对较差,检测AGR2和sTREM-1水平对评估AECOPD合并肺部感染具有重要意义.
Objective To explore the relationship of anterior gradient 2(AGR2)and soluble triggering receptor expressed on myeloid cells-1(sTREM-1)with prognosis in patients with acute exacerbation of chronic obstructive pulmonary disease(AECOPD)complicated with pulmonary infection.Methods A total of 168 AECOPD patients with pulmonary infection who were treated in Zhengzhou Central Hospital Affiliated to Zhengzhou University from October 2022 to January 2025 were prospectively selected as the infection group,and another 122 patients with AECOPD but without pulmonary infection who were matched for baseline data were included in the non-infection group.The general data and serum AGR2 and sTREM-1 levels were compared between the two groups,and the levels of serum AGR2 and sTREM-1 were compared among patients with different degrees of pulmonary infection.The predictive value of AGR2 and sTREM-1 levels on pulmonary infection in AECOPD patients was analyzed.The prognosis status was compared between both groups,and the levels of AGR2 and sTREM-1 in patients with different prognosis conditions were compared.Results The levels of AGR2 and sTREM-1[(25.31±4.40)ng/mL and(168.19±21.06)pg/mL]were significantly higher in the infection group than those in the non-infection group[(16.49±3.43)ng/mL and(149.42±16.83)pg/mL](t=19.162,8.429,P<0.001).The AGR2 and sTREM-1 levels in patients with mild infection,moderate infection and severe infection were increased successively(P<0.001).Receiver operating characteristic curve analysis revealed that the area under the curve of combined detection with AGR2 and sTREM-1 for pulmonary infection in AECOPD patients was 0.951.The good prognosis rate in the infection group[53.39%(98/165)]was significantly lower than the non-infection group[80.17%(97/121],P<0.001].Serum levels of AGR2(27.00±4.03 ng/mL)and sTREM-1(173.26±20.51 pg/mL)were significantly higher in patients with poor prognosis than in those with good prognosis[(24.53±4.28)ng/mL and(163.82±21.65)pg/mL,respectively;t=3.727,2.810,P<0.01].Conclusions Serum levels of AGR2 and sTREM-1 are significantly elevated in AECOPD patients with pulmonary infection and are associated with a poorer prognosis.Monitoring these biomarkers is of significant value for the assessment and management of such patients.
