中医药调控PI3K/AKT信号通路治疗糖尿病脑病的研究进展OA
Research Progress on Traditional Chinese Medicine Treatment of Diabetic Encephalopathy by Regulating PI3K/Akt Signaling Pathway
磷脂酰肌醇-3-激酶/蛋白激酶B(phosphatidylinositol 3-kinase/protein kinase B,PI3K/AKT)信号通路是细胞代谢的经典通路,通过调控炎症、氧化应激、细胞凋亡、突触可塑性、β-淀粉样蛋白(amyloid β-protein,Aβ)沉积及Tau蛋白过度磷酸化等机制,参与糖尿病脑病(diabetic encephalopathy,DE)的发生发展.中药单体及有效成分(如多糖类、醇苷类等)与中药复方(如七味白术散、补阳还五汤等)可通过激活PI3K/AKT通路,减轻神经炎症、抗氧化应激及抑制细胞凋亡,从而有效改善DE认知障碍.然而,相关研究仍存在局限性:机制层面多集中于PI3K/AKT信号通路活性的调控,尚未完全阐明中医药调控机制;研究多停留在细胞与动物实验阶段,缺乏临床安全性及有效性验证;干预手段以中药单体及复方为主,基于该通路的中成药及中医外治法研究较少.中药成分复杂、作用靶点多样,未来应深入探究其调控PI3K/AKT通路的具体机制,为中医药防治糖尿病脑病提供循证依据.
The phosphatidylinositol 3-kinase/protein kinase B(PI3K/Akt)signaling pathway is a classical pathway of cellular metab-olism.It participates in the occurrence and development of diabetic encephalopathy(DE)by regulating inflammation,oxidative stress,apoptosis,synaptic plasticity,amyloid β-protein(Aβ)deposition,and Tau protein hyperphosphorylation.Monomer of Chinese tradi-tional herbs and active ingredients from traditional Chinese medicine(TCM)(such as polysaccharides and glycosides)and TCM for-mulas(such as Qiwei Baizhu San and Buyang Huanwu Decoction)can alleviate neuroinflammation,counteract oxidative stress,and in-hibit apoptosis by activating the PI3K/Akt pathway,thereby effectively improving cognitive impairment in DE.However,related research still has limitations:mechanistically,studies mainly focus on the activity of the PI3K/Akt signaling pathway,and the regulatory mecha-nisms of TCM have not been fully clarified;much research remains at the cellular and animal experimental stages,with a lack of clinical safety and efficacy verification;interventions are primarily based on monomer of Chinese traditional herbs and compound formula,with fewer studies on TCM patent medicines and external therapies targeting this pathway.Due to the complexity of TCM components and the diversity of their targets,future studies should further explore the specific mechanisms by which TCM regulates the PI3K/Akt pathway,providing evidence-based guidance for the prevention and treatment of diabetic encephalopathy with TCM.
