基于"气虚"理论探讨线粒体自噬与心力衰竭的关系OA
Relationship Between Mitophagy and Heart Failure Based on"Qi Deficiency"Theory
心力衰竭作为绝大多数心血管疾病最终走向,是由于各种因素导致心脏泵血能力下降,进而全身组织供血不足而产生的一类综合征.线粒体自噬作为线粒体质量控制的一种手段,其主要目的是特异性地清除细胞中功能障碍的线粒体,从而维持细胞内稳态,让细胞发挥正常的生理功能.现代研究发现,线粒体自噬功能障碍同样会对心肌细胞的日常工作造成不利影响,并在此基础上进一步损害心脏的正常生理功能.当线粒体自噬功能产生缺陷时,心肌细胞被进一步损害.温阳益气类中药能够有效提高线粒体自噬水平,改善线粒体质量,维护心肌细胞功能.艾灸治疗和某些中药活性成分还可以抑制心肌细胞线粒体自噬持续激活,防止线粒体被过度降解而出现心功能不全.未来研究还需更进一步深挖中医药对线粒体自噬的双向调节机制,以期找到线粒体自噬障碍和过度自噬的平衡靶点,从而达到长期维持心肌线粒体稳态的效果.
Heart failure,as the ultimate outcome of most cardiovascular diseases,is a syndrome caused by various factors leading to a de-cline in the heart's pumping ability,resulting in insufficient blood supply to the whole body.Mitophagy,as a means of mitochondrial quality control,primarily aims to specifically clear dysfunctional mitochondria within cells,thereby maintaining cellular homeostasis and allowing cells to perform their normal physiological functions.Modern research has found that dysfunction of mitophagy can also adverse-ly affect the routine function of cardiomyocytes,further impairing the normal physiological function of the heart.When mitophagy is de-fective,cardiomyocytes are further damaged.Traditional Chinese medicines that functions in warming Yang and boosting Qi can effec-tively increase mitophagy levels,improve mitochondrial quality,and maintain cardiomyocyte function.Moxibustion therapy and certain active components of Chinese medicine can also inhibit the sustained activation of cardiomyocyte mitophagy,preventing mitochondria from being excessively degraded and thus avoiding cardiac dysfunction.Future research needs to further explore the bidirectional regula-tory mechanism of traditional Chinese medicine on mitophagy,with the aim of identifying balance targets between mitophagy disorder and excessive autophagy,thereby achieving long-term maintenance of mitochondrial homeostasis in cardiomyocytes.
