基于"一气周流"理论探析多囊卵巢综合征排卵障碍辨治思路OA
Analyzing the Treatment Strategies for Ovulatory Dysfunction in Polycystic Ovary Syndrome Based on the"One Qi Circulation"Theory
排卵障碍是多囊卵巢综合征(polycystic ovary syndrome,PCOS)的主要临床特征之一,亦是导致PCOS患者不孕的关键原因.基于"一气周流"理论,结合王昕教授多年临床经验,从癸水下寒、中气不运、乙木失疏、辛金难肃、丁火上炎等气机周流的关键环节,阐释PCOS排卵障碍的病因病机,并总结其治疗经验.提出以温肾水兼顾培补中焦脾土为主要原则;进而从"一气周流"角度分述治法:其一以"斡旋中轴"为主,改善卵泡质量;其二则降丁火以温癸水,促进卵泡成熟与排出;最后注重运转中轴两端之金木,以诱导排卵.该方法临床多获良效,值得借鉴.文末附临证案例1则,以供临床参考.
Ovulatory dysfunction is one of the primary clinical features of polycystic ovary syndrome(PCOS)and a key cause of infertility in PCOS patients.Based on the"One Qi Circulation"theory and integrating Professor Wang Xin's years of clinical experience,this study elucidates the etiology and pathogenesis of PCOS-related ovulatory dysfunction from the perspectives of each link in the flow of qi,such as coldness of Gui Water(Kidney Water),stagnation of Middle Qi,impaired regulation of Yi Wood(Liver Wood),difficulty in purification of Xin Metal(Lung Metal),and upward flaming of Ding Fire(Heart Fire).Furthermore,it summarizes the treatment experience,proposing the principle of warming Kidney Water while simultaneously nourishing and harmonizing the ascending and descending functions of the Spleen Earth.From the perspective of the"One Qi Circulation"theory,the treatment strategies are discussed in detail:first,focusing on regulating the central axis to improve follicle quality;second,subduing Ding Fire to warm Gui Water,thereby promoting follicle maturation and ovulation;and finally,emphasizing the regulation of Metal and Wood at both ends of the central axis to induce ovulation.These approaches have shown significant clinical efficacy and are worthy of reference.A clinical case is included for further guidance.
