新型冠状病毒感染对过敏性鼻炎和哮喘症状的短期影响:一项回顾性队列研究OA
Short-term effects of novel coronavirus disease 2019 infection on symptoms of allergic rhi-nitis and asthma:a retrospective cohort study
目的 通过回顾性队列研究,评估新型冠状病毒感染(corona virus disease 2019,COVID-19)对中国人群呼吸道过敏性疾病(包括过敏性鼻炎和过敏性哮喘)患者症状严重程度的短期影响.方法 对206例过敏性疾病患者进行回顾性问卷调查,收集其COVID-19感染情况、感染前后症状变化、用药情况及疾病控制情况.结果 COVID-19感染后,58.99%的过敏性疾病患者出现症状加重,15%报告曾出现新发过敏症状.其中,约37.7%的过敏性鼻炎患者及63.0%的过敏性哮喘患者出现过敏症状加重,包括症状评分升高、发作频率增多、症状持续时间延长,以及抗过敏药物使用频次或剂量增加;仅少数患者(过敏性鼻炎4.3%;过敏性哮喘7.6%)症状缓解或无明显变化;部分患者感染后出现新发过敏性疾病.多因素分析显示,年龄是症状加重(过敏性鼻炎:OR=1.029,95%CI:1.004~1.055,P=0.022;过敏性哮喘:OR=0.973,95%CI:0.951~0.996,P=0.023)和新发过敏性疾病(OR=0.971,95%CI:0.946~0.995,P=0.020)的重要影响因素,而疫苗接种、感染时症状和用药等无显著相关性.结论 COVID-19感染可能通过诱导免疫紊乱和气道高反应性,加重或诱发过敏性呼吸道疾病.本研究为中国人群COVID-19感染后过敏性疾病的病情变化提供了证据支持,提示临床需关注感染后过敏性疾病的评估和长期管理.
Objective To evaluate the short-term impact of coronavirus disease 2019(COVID-19)in-fection on symptom severity in Chinese patients with respiratory allergic diseases,including allergic rhinitis(AR)and allergic asthma(AA),using a retrospective cohort design.Methods A retrospective questionnaire survey was conducted among 206 patients with allergic diseases.Data were collected on COVID-19 infection sta-tus,changes in symptoms before and after infection,medication use,and disease control.Results After CO-VID-19 infection,58.99%of patients with allergic diseases experienced symptom exacerbation,and 15%re-ported newly developed allergic symptoms.Approximately 37.7%of patients with allergic rhinitis and 63.0%of patients with allergic asthma showed worsening allergic symptoms,manifested as increased symptom scores,higher attack frequency,prolonged symptom duration,and increased frequency or dosage of anti-allergic medi-cations.Only a small proportion of patients(4.3%with allergic rhinitis and 7.6%with allergic asthma)expe-rienced symptom improvement or no significant change.Additionally,some patients developed new-onset aller-gic diseases after infection.Multivariate analysis identified age as a significant factor associated with symptom exacerbation(allergic rhinitis:OR=1.029,95%CI:1.004-1.055,P=0.022;allergic asthma:OR=0.973,95%CI:0.951-0.996,P=0.023)and new-onset allergic diseases(OR=0.971,95%CI:0.946-0.995,P=0.020),whereas vaccination status,symptoms during infection,and medication use were not significantly associated.Conclusion COVID-19 infection may exacerbate or trigger allergic respiratory dis-eases by inducing immune dysregulation and airway hyperresponsiveness.This study provides retrospective evi-dence on post-COVID-19 changes in allergic disease activity in the Chinese population and highlights the need for careful post-infection assessment and long-term management of allergic diseases in clinical practice.
