慢性气道炎症性疾病是指因感染或变态反应引起的,由气道固有细胞、炎性细胞及多种细胞因子参与的慢性呼吸系统疾病,主要包括慢性阻塞性肺疾病、支气管哮喘等,其病因及发病机制复杂,目前尚未完全明确,也为临床诊疗带来了诸多挑战.中医药在诊疗慢性气道炎症性疾病中具有显著优势,但其作用机制和疗效亟待现代技术解析.代谢组学作为系统生物学的重要分支,可全面分析生物体内的代谢物网络,具有整体性、系统性和综合性等特点,与中医学整体观念、辨证论治的思维模式相契合,在识别证候特异性生物标志物、提高病证诊断和治疗方面具有重要作用,目前已广泛应用于慢性气道炎症性疾病研究中.此外,单细胞代谢组学、空间代谢组学等前沿技术的发展突破了传统代谢组学的局限,其可实现单个细胞的精准分析与检测、在组织原位上精准定位代谢物的空间分布信息等,为揭示中医药的复杂作用机制提供了新的技术支撑.该文以慢性阻塞性肺疾病和支气管哮喘为代表,系统综述近十年代谢组学及其联合多组学在中医药诊疗慢性气道炎症性疾病中的应用,并探讨其新技术在该领域的应用前景,旨在为中医药诊疗慢性气道炎症性疾病提供理论依据,为中医药现代化研究提供新的思路.

Chronic airway inflammatory diseases refer to chronic respiratory disorders caused by infections or allergic reactions,involving airway-resident cells,inflammatory cells,and various cytokines.These diseases mainly include chronic obstructive pulmonary disease and bronchial asthma.Their etiology and pathogenesis are complex and remain incompletely understood,posing numerous challenges for clinical diagnosis and treatment.TCM has demonstrated significant advantages in the diagnosis and treatment of chronic airway inflammatory diseases,yet its mechanisms of action and therapeutic efficacy urgently require elucidation through modern technologies.Metabolomics,as an important branch of systems biology,enables comprehensive analysis of metabolic networks within organisms.Characterized by its holistic,systemic,and integrative nature,it aligns well with the holistic concepts and syndrome differentiation principles of TCM.Metabolomics plays an important role in identifying syndrome-specific biomarkers and enhancing the diagnosis and treatment of diseases,and it has now been widely applied in research on chronic airway inflammatory diseases.Furthermore,advances in frontier technologies such as single-cell metabolomics and spatial metabolomics have overcome the limitations of traditional metabolomics.These technologies enable precise analysis and detection at the single-cell level,as well as in situ spatial localization of metabolites within tissues,thereby providing new methodological support for elucidating the complex mechanisms of TCM.Using chronic obstructive pulmonary disease and bronchial asthma as representative conditions,this review systematically summarizes the application of metabolomics and its integration with multi-omics approaches in the TCM-based diagnosis and treatment of chronic airway inflammatory diseases over the past decade.It also explores the prospects of these emerging technologies in this field,with the aim of providing a theoretical foundation for TCM diagnosis and treatment of chronic airway inflammatory diseases and offering new perspectives for the modernization of TCM research.
