目的 探讨AI图像后处理技术对双低(80 kVp管电压、40 ml对比剂)冠状动脉CT血管成像(coronary computed tomography angiography,CCTA)图像质量的影响.方法 本研究回顾性纳入102例2021年1月至2023年10月在海安市中医院行CCTA检查且BMI≤25 kg/m2 的患者,通过随机数表法随机分为100 kVp组及80 kVp组,通过传统后处理方法和AI算法重建图像,对冠状动脉进行主观及客观图像质量评估.比较组间一般资料、图像质量主观及客观评估结果、辐射剂量的差异.结果 本研究共纳入102例患者,其中100 kVp组53例,80 kVp组49例.两组患者客观图像质量比较:100 kVp组平均冠状动脉CT值[(527.00±80.00)HU比(343.00±60.00)HU,P<0.001]、图像噪声[(26.00±7.00)HU比(21.00±7.00)HU,P=0.002]及信噪比(20.30±4.20比16.30±4.30,P<0.001)均明显高于80 kVp组,但两组图像对比噪声比差异无统计学意义(24.50±4.00和22.60±6.20,P=0.099).采用AI与传统后处理方法,两组间主观图像质量评分差异均无统计学意义.80 kVp组辐射剂量显著低于100 kVp组.结论 双低CCTA可以降低患者辐射剂量,且采用AI重建图像可以在保证冠状动脉影像质量的前提下,提高医生的工作效率.

Objective To explore the influence of artificial intelligence(AI)-based post-processing technology on the image quality in double low(80 kVp tube voltage,40 ml contrast agent)coronary computed tomography angiography(CCTA).Methods This retrospective study enrolled patients who underwent CCTA examinations at Hai'an Hospital of Traditional Chinese Medicine between January 2021 and October 2023,with a body mass index(BMI)≤25 kg/m2.Patients were randomly assigned to either the 100 kVp group or the 80 kVp group using a random number table.Images were reconstructed using both conventional post-processing methods and AI algorithms,and subjective and objective image quality assessments of the coronary arteries were performed.Differences in general characteristics,subjective and objective image quality assessments,and radiation dose between groups were compared.Results A total of 102 patients were enrolled in this study,including 53 patients in the 100 kVp group and 49 patients in the 80 kVp group.The 100 kVp group demonstrated significantly higher average coronary artery CT values[(527.00±80.00)HU vs.(343.00±60.00)HU,P<0.001],image noise[(26.00±7.00)HU vs.(21.00±7.00)HU,P=0.002)],and signal-to-noise ratio(SNR)[(20.30±4.20)vs.(16.30±4.30),P<0.001]compared to the 80 kVp group.However,there was no statistically significant difference in contrast-to-noise ratio(CNR)between the two groups[(24.50±4.00)vs.(22.60±6.20),P=0.099].There was no statistically significant difference in subjective image quality scores between images processed using artificial intelligence(AI)and traditional post-processing methods.The radiation dose in the 80 kVp group was significantly lower than that in the 100 kVp group.Conclusions Double low CCTA can reduce the radiation dose for patients,and AI-reconstructed images enhance physician workflow efficiency while maintaining coronary artery image quality.
