老年听障患者心理健康状况及其影响因素分析OA
Mental Health Status and Influencing Factors in Elderly Patients with Hearing Impairment
目的 分析老年听障患者心理健康状况及其影响因素.方法 选取老年听障患者224例,采用症状自评量表(symptom checklist 90,SCL-90)评估其心理健康状况,分为阳性组、阴性组;采用医疗社会支持量表(medical outcomes study social support,MOS-SSS)评估其接受社会支持情况,对比阳性组、阴性组接受社会支持情况的差异.收集两组患者临床资料,采用Logistic回归模型分析危险因素,经回归方程拟合的概率值绘制ROC曲线.结果 SCL-90阳性组126例(56.25%),阴性组98例(43.75%).阳性组MOS-SSS总分显著低于阴性组(P＜0.05).Logistic回归显示,未婚/离婚/丧偶、家庭人均年收入<3万元、独居、合并长期疾病3种及以上、听力障碍分级重度或极重度是老年听障患者心理健康情况的独立危险因素(P＜0.05),MOS-SSS总分是其保护因素(P＜0.05).ROC曲线分析显示,联合预测的AUC为0.980,95%CI为0.960～1.000,灵敏度为0.905,特异度为0.908.结论 56.25%的老年听障患者存在心理问题.婚姻状况、家庭人均年收入、居住情况、合并慢性疾病、听力障碍分级、MOS-SSS总分是其健康情况的重要影响因素,联合预测效能较高.应重点关注具有危险因素的老年听障患者,针对性改善其心理健康状况.
Objective To analyze the mental health status and influencing factors in elderly patients with hearing impairment.Methods 224 elderly hearing-impaired patients were selected.Their mental health status was evaluated using the Symptom Checklist 90(SCL-90),and they were divided into the positive group and the negative group based on the results.The Medical Outcomes Study Social Support Scale(MOS-SSS)was used to assess the social support received by the elderly hearing-impaired patients,and the social support received by the positive group and the negative group was compared.The clinical data of the two groups of patients were collected,and the risk factors analyzed using the Logistic regression model.The ROC curve was plotted based on the probability values obtained from the regression equation.Results SCL-90 positive 126 cases(56.25%),classified as positive group.98 cases(43.75%)were negative,classified as the negative group.The total score of MOS-SSS in the positive group was significantly lower than that in the negative group(P<0.05).Logistic regression analysis showed that unmarried/divorced/widowed,family per capita annual income<30000,living alone,combined with 3 or more long-term diseases,severe or extremely severe hearing impairment were independent risk factors for mental health in elderly patients with hearing impairment(P<0.05).The total score of MOS-SSS was its protective factor(P<0.05).ROC curve analysis showed that the AUC of combined prediction was 0.980,95%CI was 0.960-1.000,sensitivity 0.905,and specificity 0.908.Conclusion 56.25%of elderly patients with hearing impairment have mental health problems.Marital status,family per capita annual income,living situations,number of chronic diseases,hearing impairment degrees,and MOS-SSS total score are important influencing factors for the mental health status of elderly patients with hearing impairment,with high combined predictive efficacy.It is essential to focus on elderly patients with hearing impairment who have these risk factors and provide targeted interventions to improve their mental health status.
