紧密型医联体的"嵌入"机制构建:理论逻辑与路径创新OA
Constructing relational embedding mechanisms for close-knit medical consortia:Theoretical logic and path innovation
当前医联体建设稳步推进,有效推动了优质医疗资源下沉和均衡分布,但也不同程度呈现管理体制碎片化、利益分配机制不健全、医疗资源流动不畅、服务同质化不足、政策配套滞后、文化认同与协作不足等影响和制约紧密型医联体建设的制度性、技术性、文化性障碍问题.嵌入性理论为分析组织间合作提供了独特视角,揭示混合型合作模式(如医联体)中社会资本的作用;解释社会网络中的信任与互惠比契约更能维系长期协作;通过非正式协调机制能够弥补或化解正式制度的刚性缺陷.关系嵌入型医联体机制构建路径:认知嵌入——确立命运与共的价值共识;关系嵌入——打造衔接有序的分诊流程;结构嵌入——形成功能互补的发展格局;制度嵌入——构建权责明晰的行为路径.
At present,the construction of medical consortia is steadily advancing,effectively promoting the equitable allocation and balanced distribution of high-quality medical resources.However,institutional,tech-nical,and cultural barriers persist in the construction of close-knit medical consortia,including fragmented man-agement systems,imperfect benefit-sharing mechanisms,constrained medical resource mobility,insufficient serv-ice standardization,delayed policy implementation,and inadequate cultural identity and collaboration.Embedded-ness theory provides a unique analytical framework for inter-organizational collaboration,revealing:(1)the pivotal role of social capital in hybrid cooperative models(e.g.,medical consortia);(2)that trust-based reciprocity in social networks is more effective than formal contracts in sustaining long-term cooperation;and(3)how informal coordination mechanisms can compensate for or mitigate the rigidity of formal institutions.The proposed relational embedding mechanism follows a fourfold implementation pathway:cognitive embedding by establishing value con-sensus of shared destiny,relational embedding by developing well-coordinated triage protocols,structural embed-ding by forming complementary functional configurations and institutional embedding by defining clear rights and responsibilities.
