Preventive and therapeutic effects of pneumatic compression therapy on deep vein thrombosis after hip and knee re-placement surgery in elderly patients with osteoarthritis

目的:探讨气压疗法在老年骨性关节炎(osteoarthritis,OA)髋膝关节置换术后治疗中的运用及其对深静脉血栓(deep vein thrombosis,DVT)的防治作用及安全性.方法:自2022年4月至2024年4月,采取髋膝关节置换术治疗100例老年OA患者,按治疗方法不同分为对照组和研究组.对照组50例采取常规综合疗法,男26例,女24例;年龄60~80(69.62±7.13)岁;OA病程9~19(14.78±3.64)年;患侧为左侧17例,右侧17例,双侧16例.研究组50例在对照组常规防治基础上应用气压疗法干预,男25例,女25例;年龄61~80(68.92±6.48)岁;OA病程9~20(14.54±4.72)年;患侧为左侧18例,右侧17例,双侧15例.两组均连续治疗7d.观察记录两组术后DVT发生率、下肢肿胀情况;在治疗前后检测患肢股总静脉血流速度、凝血功能指标;观察治疗期间不良反应.结果:术后所有患者切口 Ⅰ期愈合.100例获得随访,时间12~24(16.85±3.11)个月.研究组术后7 d的DVT发生低于对照组,差异有统计学意义(P<0.05);两组下肢肿胀分级比较,差异有统计学意义(P<0.05).两组治疗后股总静脉、腘静脉血流速度快于治疗前,差异有统计学意义(P<0.05);且研究组快于对照组,差异有统计学意义(P<0.05).与治疗前比较,两组治疗后凝血酶原时间(prothombin time,PT)、活化部分凝血活酶时间(activited partial thomboplastin time,APTT)均明显延长,差异有统计学意义(P<0.05);纤维蛋白原(fibrinogen,FIB)、D二聚体(D-dimer,D-D)均明显降低,差异有统计学意义(P<0.05);其中研究组PT、APTT长于对照组,差异有统计学意义(P<0.05);研究组FIB、D-D低于对照组,差异有统计学意义(P<0.05).两组不良反应发生率比较,差异无统计学意义(P>0.05).结论:气压疗法对老年OA髋膝关节置换术后DVT的防治效果显著,可加快血流速度、调节凝血功能,安全可靠,但在临床应用中需考虑出血倾向、皮肤破损等禁忌证,避免发生风险.

Objective To investigate the application of pneumatic compression therapy in elderly patients with osteoarthri-tis(OA)undergoing hip and knee replacement surgery,and to evaluate its efficacy and safety in the prevention and treatment of deep vein thrombosis(DVT).Methods From April 2022 to April 2024,100 elderly OA patients who underwent hip or knee replacement surgery were enrolled.They were divided into a control group and a study group according to different treatment methods.Among them,50 patients in the control group received conventional comprehensive therapy,including 26 males and 24 females,aged 60 to 80 years with a mean of(69.62±7.13)years;OA duration was 9 to 19 years with a mean of(14.78±3.64)years;affected side:left 17 cases,right 17 cases,bilateral 16 cases.Other 50 patients in the study group received pneu-matic compression therapy in addition to conventional prevention and treatment,including 25 males and 25 females,aged 61 to 80 years with a mean of(68.92±6.48)years;OA duration was 9 to 20 years with a mean(14.54±4.72)years;affected side:left 18 cases,right 17 cases,bilateral 15 cases.Both groups were treated continuously for 7 days.The incidence of postoperative DVT and lower limb swelling were recorded.The femoral vein blood flow velocity and coagulation parameters were measured before and after treatment.Adverse reactions during treatment were observed.Results All patients achieved primary wound healing.All 100 patients were followed up for 12 to 24 months with a mean of(16.85±3.11)months.The incidence of DVT in the study group at 7 days after surgery was significantly lower than that in the control group(P<0.05).There was a significant difference in lower limb swelling grade between the two groups(P<0.05).After treatment,the blood flow velocities of the femoral and popliteal veins in both groups were significantly higher than before treatment(P<0.05),and the study group showed higher velocities than the control group(P<0.05).Compared with before treatment,prothombin time(PT)and activited partial thomboplastin time(APTT)were significantly prolonged(P<0.05),while fibrinogen(FIB)and D-dimer were significantly de-creased(P<0.05)in both groups after treatment.The study group showed longer PT and APTT and lower FIB and D-dimer lev-els than the control group(P<0.05).There was no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusion Pneumatic compression therapy is effective in preventing and treating DVT after hip and knee replacement surgery in elderly OA patients,as it accelerates blood flow velocity and improves coagulation function.It is safe and reliable,but contraindications such as bleeding tendency and skin damage should be considered in clinical application to avoid potential risks.
