磁共振弥散加权成像结合常规序列在老年急性脑梗死诊断中的价值研究OA
Value of Magnetic Resonance Diffusion-Weighted Imaging Combined with Conventional Sequences in the Diagnosis of Acute Cerebral Infarction in Elderly Patients
目的 探讨磁共振(MRI)弥散加权成像(DWI)与常规序列联合诊断老年急性脑梗死的效能.方法 回顾性选取驻马店市中医院 2023 年 3 月至 2025 年 3 月收治的 102 例疑似急性脑梗死的老年患者作为研究对象,以最终临床诊断结果为"金标准",诊出阳性 70 例、阴性 32 例.分析DWI、常规序列单独及联合对急性脑梗死的诊断效能,比较DWI和常规序列单独及联合对急性脑梗死不同时期的检出率.结果 DWI诊出阳性58例(56.86%),阴性44例(43.14%),常规序列诊出阳性 52 例(50.98%),阴性 50 例(49.02%),DWI联合常规序列诊出阳性 77 例(75.49%),阴性 25例(24.51%);DWI联合常规序列诊断急性脑梗死的灵敏度为 95.71%,高于DWI的 72.86%和常规序列的 61.43%;联合诊断的漏诊率 4.29%,显著低于二者单独诊断的 27.14%、38.57%,差异均有统计学意义(P<0.05);DWI联合常规序列对超急性期、急性期检出率显著高于二者单独诊断(P<0.05).结论 MRI的DWI与常规序列联合应用可显著提高对老年急性脑梗死的诊断效能.同时,联合DWI与常规序列可提高对超急性期、急性期脑梗死的检出率,有助于明确早期诊断,为临床及时干预提供可靠依据.
Objective To investigate the diagnostic performance of magnetic resonance imaging(MRI)diffusion-weighted imaging(DWI)combined with conventional sequences for acute cerebral infarction in elderly patients.Methods A total of 102 elderly patients with suspected acute cerebral infarction who were admitted to Zhumadian Hospital of Traditional Chinese Medicine from March 2023 to March 2025 were retrospectively selected as the study subjects.Using the final clinical diagnosis as the gold standard,70 patients were confirmed positive and 32 were confirmed negative.The diagnostic performance of DWI alone,conventional sequences alone,and their combination for acute cerebral infarction was analyzed,and the detection rates of DWI alone,conventional sequences alone,and their combination for acute cerebral infarction at different stages were compared.Results DWI detected 58 positive cases(56.86%)and 44 negative cases(43.14%);conventional sequences detected 52 positive cases(50.98%)and 50 negative cases(49.02%);DWI combined with conventional sequences detected 77 positive cases(75.49%)and 25 negative cases(24.51%).The sensitivity of DWI combined with conventional sequences for diagnosing acute cerebral infarction was 95.71%,which was higher than that of DWI alone(72.86%)and conventional sequences alone(61.43%).The missed diagnosis rate of the combined diagnosis was 4.29%,which was significantly lower than 27.14%for DWI alone and 38.57%for conventional sequences alone,and the differences were statistically significant(P<0.05).The detection rates of DWI combined with conventional sequences in the hyperacute stage and acute stage were significantly higher than those of either modality used alone(P<0.05).Conclusion The combined application of MRI DWI and conventional sequences can significantly improve the diagnostic performance for acute cerebral infarction in elderly patients.In addition,the combination of DWI and conventional sequences can improve the detection rates of cerebral infarction in the hyperacute and acute stages,which helps to establish an early diagnosis and provides a reliable basis for timely clinical intervention.
