胃肠充盈超声造影对胃溃疡疾病的诊断效能研究OA

Objective To investigate the diagnostic efficacy of gastrointestinal filling contrast-enhanced ultrasonography in gastric ulcer disease.Methods A retrospective analysis was conducted on the medical records of 80 patients with suspected gastric ulcer who underwent gastrointestinal filling contrast-enhanced ultrasonography at Beijing Zhongguancun Hospital from January 2023 to May 2025.Gastroscopy findings were used as the gold standard to analyze the diagnostic value of gastrointestinal filling contrast-enhanced ultrasonography for gastric ulcer.According to the gastroscopy findings,the patients were divided into a negative group and a positive group,and the manifestations of gastrointestinal filling contrast-enhanced ultrasonography were compared between the two groups.Receiver operating characteristic(ROC)curves were plotted to evaluate the sensitivity and specificity of quantitative gastrointestinal filling ultrasonographic parameters,including gastric wall thickness(GWT),resistance index(RI),and peak systolic velocity(PSV),for the diagnosis of gastric ulcer.Results The consistency between gastrointestinal filling contrast-enhanced ultrasonography and gastroscopy in diagnosing gastric ulcer lesions was acceptable,with a Kappa value of 0.698(P＜0.05).The sensitivity,specificity,and accuracy of gastrointestinal filling contrast-enhanced ultrasonography for diagnosing gastric ulcer were 82.61%,88.24%,and 85.00%,respectively.The proportions of imaging signs such as regular lesion morphology and clear gastric wall stratification in the lesion area were higher in the negative group than in the positive group.The GWT and PSV in the negative group were lower than those in the positive group,whereas the RI in the negative group was higher than that in the positive group(P＜0.05).The AUC of GWT for diagnosing gastric ulcer was 0.816(95%CI:0.743～0.896),the AUC of RI was 0.889(95%CI:0.802～0.985),and the AUC of PSV was 0.723(95%CI:0.638～0.819)(P＜0.05).Conclusion Gastrointestinal filling contrast-enhanced ultrasonography shows acceptable consistency with gastroscopy in the diagnosis of gastric ulcer and has good diagnostic efficacy.
