Research progress of traditional Chinese medicine in regulating the Keap1/Nrf2/ARE signaling pathway in the treatment of Alzheimer's disease

阿尔茨海默病(AD)作为一种慢性神经退行性疾病,现有药物治疗效果有限.中医药在改善临床症状与延缓病程方面展现出独特优势,其作用机制与调控kelch样ECH相关蛋白1(Keap1)/核因子NF-E2相关因子2(Nrf2)/抗氧化反应元件(ARE)信号通路密切相关.文章系统综述了Keap1/Nrf2/ARE通路在AD中的神经保护作用,包括减轻氧化应激、抑制神经炎症、改善线粒体功能、调节铁代谢、抑制细胞凋亡以及调控自噬等方面,并进一步探讨了中药单体成分、大类成分、复方制剂及中医技术通过调控该通路治疗AD的潜在机制,以期为中医药在AD防治中的临床应用及新药研发提供理论参考.

Alzheimer's disease(AD)is a chronic neurodegenerative disease with limited treatment options.Traditional Chinese medi-cine(TCM)has demonstrated potential in alleviating clinical symptoms and delaying disease progression.The therapeutic mechanisms of TCM are closely associated with the regulation of the Keap1)/Nrf2/ARE signaling pathway.This review systematically summarizes the neuroprotective effects of the Keap1/Nrf2/ARE pathway in AD,which include mitigating oxidative stress,inhibiting neuroinflammation,enhancing mitochondrial function,regulating iron metabolism,suppressing apoptosis,and modulating autophagy.Furthermore,it eluci-dates how Chinese medicinal(CM)-derived active monomers,major constituents,compound formulations,and related non-pharmacological therapies exert their effects against AD through modulating this pathway.This review aims to provide a theoretical foun-dation for the clinical application of TCM and the development of novel medicinal agents for AD.
