Association between Serum Uric Acid to High-Density Lipoprotein Cholesterol Ratio and Metabolic Syndrome in Young and Middle-Aged Populations:a Cross-Sectional Study

目的 探讨血尿酸/高密度脂蛋白胆固醇比值(UHR)与青年和中老年人群发生代谢综合征(MS)的关系.方法 选取2021年12月—2022年1月于宿州市埇桥区东关街道社区卫生服务中心进行健康体检的1 066名体检者为研究对象,收集所有受试者的临床资料,根据年龄将其分为青年组(年龄<45岁)和中老年组(年龄≥45岁),根据是否诊断为MS将青年组受试者分为MS1亚组和非MS1亚组,将中老年组受试者分为MS2亚组和非MS2亚组.采用多因素Logistic回归分析探讨青年组及中老年组发生MS的影响因素,采用ROC曲线分析探讨UHR对青年组及中老年组发生MS的预测价值.结果 1 066例受试者中青年组631例,中老年组435例.中老年组MS检出率高于青年组,血尿酸、UHR低于青年组(P<0.05).非MS1亚组493例,MS1亚组128例.MS1亚组男性占比、有吸烟史者占比、有饮酒史者占比、体质指数(BMI)、收缩压、舒张压、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、空腹血糖(FBG)、血肌酐、血尿酸、UHR高于非MS1亚组,年龄、腰围大于非MS1亚组,大专及以上者占比和高密度脂蛋白胆固醇(HDL-C)低于非MS1亚组,肱踝脉搏波传导速度(baPWV)快于非MS1亚组(P<0.05).非MS2亚组291例,MS2亚组144例.MS2亚组有吸烟史者占比、BMI、收缩压、舒张压、TG、FBG、血肌酐、血尿酸、UHR高于非MS2亚组,腰围大于非MS2亚组,HDL-C低于非MS2亚组(P<0.05).多因素Logistic回归分析结果显示,年龄、吸烟史、BMI、TG、TC、LDL-C、FBG、UHR是青年组发生MS的独立影响因素(P<0.05),BMI、腰围、TG、FBG、UHR是中老年组发生MS的独立影响因素(P<0.05).ROC曲线分析结果显示,UHR预测青年组发生MS的AUC为0.872[95%CI(0.839~0.905)],UHR预测中老年组发生MS的AUC为0.800[95%CI(0.754~0.846)],UHR预测青年组发生MS的AUC大于UHR预测中老年组发生MS的AUC(P<0.05).结论 中老年人群MS检出率高于青年人群,UHR低于青年人群.UHR是青年人群及中老年人群发生MS的独立影响因素,且UHR预测青年人群发生MS的价值更高.

Objective To investigate the association between serum uric acid to high-density lipoprotein cholesterol ratio(UHR)and metabolic syndrome(MS)in young and middle-aged populations.Methods A total of 1 066 individuals who underwent health examinations at the Dongguan Community Health Service Center in Yongqiao District,Suzhou City from December 2021 to January 2022 were selected as the research subjects.The clinical data of the subjects were collected,and they were divided into a youth group(<45 years old)and a middle-aged and elderly group(≥45 years old)based on age.The subjects of the youth group were divided into the MS1 subgroup and the non-MS1 subgroup according to whether MS was diagnosed,and the subjects of the middle-aged and elderly group were divided into the MS2 subgroup and the non-MS2 subgroup.Multivariate Logistic regression analysis was used to explore the influencing factors of MS in the youth group and the middle-aged and elderly group.The ROC curve was used to explore the predictive value of the UHR for MS in the youth group and the middle-aged and elderly group.Results Among the 1 066 subjects,631 in the young group and 435 in the middle-aged and elderly group.The MS detection rate in the middle-aged and elderly group was higher than that in the youth group,serum uric acid and UHR were lower than those in the youth group(P<0.05).Four hundred and ninety-three subjects were in the non-MS1 subgroup,138 subjects were in the MS1 subgroup.The proportion of males,the proportion of people with smoking history,the proportion of people with drinking history,body mass index(BMI),systolic blood pressure,diastolic blood pressure,triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),fasting blood glucose(FBG),blood creatinine,blood uric acid,UHR in the MS1 subgroup were higher than those in the non-MS1 subgroup,age,waist circumference were greater than those in the non-MS1 subgroup,the proportion of people with junior college and above,high density lipoprotein cholesterol(HDL-C)were lower than those in the MS1 subgroup,brachial-ankle pulse wave velocity(baPWV)was faster than that in the non-MS1 subgroup(P<0.05).Two hundred and ninety-one subjects were in the non-MS2 subgroup,144 subjects were in the MS2 subgroup.The proportion of people with smoking history,BMI,systolic blood pressure,diastolic blood pressure,TG,FBG,blood creatinine,blood uric acid,UHR in the MS2 subgroup were higher than those in the non-MS2 subgroup,waist circumference was greater than that in the non-MS2 subgroup,while the HDL-C was lower than that in the non-MS2 group(P<0.05).Multivariate Logistic regression analysis showed that age,smoking history,BMI,TG,TC,LDL-C,FBG,UHR were independent influencing factors of MS in the youth group(P<0.05),BMI,waist circumference,TG,FBG,UHR were independent influencing factors of MS in the middle-aged and elderly group(P<0.05).ROC curve analysis showed that the AUC of UHR in predicting MS in the youth group was 0.872[95%CI(0.839-0.905)],the AUC of UHR in predicting MS in the middle-aged and elderly group was 0.800[95%CI(0.754-0.846)].The AUC of UHR in predicting MS in the youth group was higher than the AUC of UHR in predicting MS in the middle-aged and elderly group(P<0.05).Conclusion The MS detection rate in the middle-aged and elderly population is higher than that in the young population,and UHR is lower than that in the young population.UHR is an independent influencing factor of MS in the young population and the middle-aged and elderly population,and it holds greater predictive value for MS in the young population.
