目的 探讨尿酸与白蛋白比值(UAR)与高血压患者左心室肥厚的关系.方法 回顾性选取2021年9月—2025年1月安徽医科大学第一附属医院收治的高血压患者438例为研究对象.收集患者的临床资料,根据是否合并左心室肥厚[男性左心室质量指数(LVMI)≥109 g/m2或女性LVMI≥105 g/m2]将患者分为肥厚组(n=158)和非肥厚组(n=280).采用Spearman秩相关分析探讨高血压患者UAR与LVMI的相关性;影响因素分析采用单因素及多因素Logistic回归分析.结果 肥厚组年龄大于非肥厚组,收缩压、脉压、高血压病程≥10年者占比、血肌酐、尿酸、空腹血糖、UAR高于非肥厚组,白蛋白低于非肥厚组(P<0.05).Spearman秩相关分析结果显示,高血压患者UAR与LVMI呈正相关(rs=0.275,P<0.001).单因素Logistic回归分析结果显示,年龄、收缩压、脉压、高血压病程、血肌酐、空腹血糖、UAR可能是高血压患者合并左心室肥厚的影响因素(P<0.05).多因素Logistic回归分析结果显示,脉压、高血压病程、UAR是高血压患者合并左心室肥厚的独立影响因素(P<0.05).结论 高血压患者UAR与LVMI呈正相关,UAR是高血压患者合并左心室肥厚的独立影响因素.

Objective To investigate the relationship between uric acid to albumin ratio(UAR)and left ventricular hypertrophy in patients with hypertension.Methods A total of 438 patients with hypertension admitted to the First Affiliated Hospital of Anhui Medical University from September 2021 to January 2025 were retrospectively selected as the research subjects.The clinical data of the patients were collected,and the patients were divided into hypertrophy group(n=158),and non-hypertrophy group(n=280)according to the presence of left ventricular hypertrophy[left ventricular mass index(LVMI)≥109 g/m2 for males or LVMI≥105 g/m2 for females].Spearman rank correlation analysis was used to explore the correlation between UAR and LVMI in patients with hypertension.Univariate and multivariate Logistic regression analysis was used to explore the influencing factors.Results The age in the hypertrophy group was older than that in the non-hypertrophy group,systolic blood pressure,pulse pressure,proportion of patients with hypertension duration≥10 years,serum creatinine,uric acid,fasting blood glucose,and UAR were higher than those in the non-hypertrophy group,albumin was lower than that in the non-hypertrophy group(P<0.05).Spearman rank correlation analysis showed that there was a positive correlation between UAR and LVMI in patients with hypertension(rs=0.275,P<0.001).Univariate Logistic regression analysis showed that age,systolic blood pressure,pulse pressure,hypertension duration,serum creatinine,fasting blood glucose,and UAR may be influencing factors of left ventricular hypertrophy in patients with hypertension(P<0.05).Multivariate Logistic regression analysis showed that pulse pressure,hypertension duration,and UAR were independent influencing factors of left ventricular hypertrophy in patients with hypertension(P<0.05).Conclusion There is a positive correlation between UAR and LVMI in patients with hypertension,and UAR is an independent influencing factor of left ventricular hypertrophy in patients with hypertension.
