踝肱指数与老年高血压患者发生心脑血管死亡的关系研究OA
Relationship between Ankle-Brachial Index and Incidence of Cardio-Cerebrovascular Death in Elderly Hypertensive Patients
目的 探讨踝肱指数(ABI)与老年高血压患者发生心脑血管死亡的关系.方法 选取2021年3-6月于绵阳市中心医院接受治疗的老年高血压患者1 202例作为研究对象.收集患者的临床资料.随访3年,根据心脑血管死亡发生情况将患者分为心脑血管死亡组与无心脑血管死亡组.采用广义相加模型分析ABI与老年高血压患者心脑血管死亡发生风险的相关性.采用多因素Cox比例风险回归分析探讨ABI与老年高血压患者发生心脑血管死亡的关系.结果 随访3年,剔除ABI数据缺失者516例、失访者6例,最终纳入680例.680例患者中,发生心脑血管死亡43例(6.32%).心脑血管死亡组吸烟率、饮酒率、合并慢性肾脏病者占比高于无心脑血管死亡组,三酰甘油、低密度脂蛋白胆固醇、ABI低于无心脑血管死亡组(P<0.05).广义相加模型分析结果显示,ABI与老年高血压患者心脑血管死亡发生风险呈"L"型非线性相关(P整体=0.023,P非线性=0.005),最低拐点为1.16.多因素Cox比例风险回归分析结果显示,ABI<1.16是老年高血压患者发生心脑血管死亡的独立危险因素(P<0.001).结论 ABI与老年高血压患者心脑血管死亡发生风险呈"L"型非线性相关,ABI<1.16是老年高血压患者发生心脑血管死亡的独立危险因素.
Objective To investigate the relationship between ankle-brachial index(ABI)and incidence of cardio-cerebrovascular death in elderly hypertensive patients.Methods A total of 1 202 patients with hypertension who received treatment at Mianyang Central Hospital from March to June 2021 were selected as the study participants.Clinical data of the patients were collected.After 3 years of follow-up,patients were divided into the cardio-cerebrovascular death group and the non-cardio-cerebrovascular death group according to the occurrence of cardio-cerebrovascular death.Generalized additive model was used to analyze the correlation between ABI and the risk of cardio-cerebrovascular death in elderly hypertensive patients,and multivariate Cox proportional hazards regression analysis was used to analyze the relationship between ABI and the risk of cardio-cerebrovascular death in elderly hypertensive patients.Results After 3 years of follow-up,516 patients with missing ABI data and 6 patients lost to follow-up were excluded,ultimately 680 patients were enrolled.Among the 680 patients,43 cases(6.32%)happened cardio-cerebrovascular death.The smoking rate,drinking rate,proportion of patients with chronic kidney disease in the cardio-cerebrovascular death group were higher than those in the non-cardio-cerebrovascular death group,triglyceride,low-density lipoprotein cholesterol,and ABI were lower than those in the non-cardio-cerebrovascular death group(P<0.05).Generalized additive model analysis results revealed that ABI exhibited a"L"-shaped nonlinear correlation with cardio-cerebrovascular death in elderly hypertensive patients(P for overall=0.023,P for nonlinear=0.005),and with the lowest inflection point at 1.16.Multivariate Cox proportional hazards regression analysis results showed that ABI<1.16 was an independent risk factor for cardio-cerebrovascular death in elderly hypertensive patients(P<0.001).Conclusion ABI exhibits a"L"-shaped nonlinear correlation with cardio-cerebrovascular death in elderly hypertensive patients,and ABI<1.16 is an independent risk factor for cardio-cerebrovascular death in patients with hypertension.
