针刺调控脑肠轴改善卒中后便秘的多机制协同作用OA

卒中后便秘(PSC)作为脑卒中常见并发症,发生率较高,在急性期约为45%,康复期约为48%.其不仅会引发腹胀、腹痛等不适,影响患者生活质量与康复进程,还可能导致肺炎、复发性中风等并发症,甚至因患者用力排便增加脑疝风险,危及生命.目前,药物治疗PSC存在依赖性、肠道功能恶化等不良反应,而针刺作为绿色安全的中医疗法,展现出显著优势.脑肠轴(GBA)作为中枢神经系统与胃肠道间的双向调节网络,为针刺治疗提供了新靶点.文章提出针刺通过"神经-内分泌-菌群"三级调控网络改善PSC的整合假说.在神经系统调节方面,针刺可影响中枢神经系统(如上调BDNF表达增强肠道动力)、肠神经系统(如增加肠神经丛数量、清除异常聚集的α-突触核蛋白)及自主神经系统(如激活迷走神经促进胃肠传导);对脑肠肽的调节上,能升高兴奋性肽(如SP、5-HT)、降低抑制性肽(如SS、VIP),改善胃肠动力;在肠道菌群调节中,可降低大肠杆菌丰度、增加有益菌,维持菌群平衡;还能调节下丘脑-垂体-肾上腺轴(HPA轴),纠正其功能紊乱,缓解内脏超敏反应.各机制相互关联,形成"神经先导-激素协同-菌群稳态"的级联反应,首次揭示了针刺改善PSC的多系统协同作用,为中西医结合治疗PSC提供了理论范式与新思路.未来研究可聚焦针刺信号转导路径、迷走神经监测及"穴位-菌群"关系等方向,以深入阐释其作用机制.

As a common complication of stroke,post-stroke constipation(PSC)has a high incidence rate of about 45%in the acute stage and 48%in the rehabilitation stage.It not only causes abdominal distension,abdominal pain and other discomforts,which affects patients'quality of life and recovery process,but also may lead to complications such as pneumonia,recurrent stroke,and even increase the risk of brain herniation due to the patient's straining to defecate,which is life-threatening.Currently,drug treatment for PSC has side effects such as dependence and deterioration of intestinal function,while acupuncture,as a green and safe Chinese medicine therapy,shows significant advantages.The brain-gut axis(GBA),as a bidirectional regulatory network between the central nervous system and the gastrointestinal tract,provides a new target for acupuncture therapy.In this paper,we propose the integration hypothesis that acupuncture improves PSC through the"neuro-endocrine-flora"tertiary regulatory network.In terms of nervous system regulation,acupuncture can affect the central nervous system(e.g.,up-regulate BDNF expression to enhance intestinal motility),enteric nervous system(e.g.,increase the number of enteric plexus,remove abnormal aggregation of α-synaptic nucleoprotein),and the autonomic nervous system(e.g.,activate the vagus nerve to promote gastrointestinal conduction);in terms of the regulation of cerebral and intestinal peptides,it can elevate excitatory peptides(e.g.,SP,5-HT),reduce inhibitory peptides(e.g.,SS,VIP),and improve gastrointestinal conduction.and VIP),improving gastrointestinal dynamics;in the regulation of intestinal flora,it can reduce the abundance of Escherichia coli,increase the beneficial bacteria,and maintain the balance of bacterial flora;it can also regulate the hypothalamus-pituitary-adrenal axis(HPA axis),correcting the dysfunction of its function,and alleviating the hypersensitivity reaction of internal organs.These mechanisms are interrelated,forming a cascade of"neural guidance-hormonal synergy-bacterial homeostasis",which reveals for the first time the synergistic effect of acupuncture in improving PSC,and provides a theoretical paradigm and a new way of thinking for the treatment of PSC by combining traditional Chinese and Western medicine.Future studies may focus on the signaling pathway of acupuncture,vagus nerve monitoring and"acupoint-colony"relationship,in order to further elucidate its mechanism of action.
