目的 探讨血浆成纤维细胞生长因子(FGF)-21水平与社区老年人内在能力(IC)的相关性.方法 2023年2-8月对北京市4个社区内671例年龄≥60岁的老人进行横断面调查,收集一般人口特征、体格检查结果和血浆标本.根据2015年世界卫生组织推荐意见评估IC,分为两组:IC正常(9~10分)和IC下降(0~8分);采用酶联免疫吸附法检测受试者血浆中的FGF-21浓度,按照三分位数分为高、中、低三组.使用Spearman相关系数分析FGF-21水平与IC评分的相关性;使用多因素logistic回归和受试者操作特征(ROC)曲线分析FGF-21水平对IC下降的影响及识别能力.结果 249例(37.11%)存在IC下降,IC正常组的 FGF-21 水平显著高于 IC 下降组[3.23(2.78,3.88)ng/L 比 2.81(2.48,3.31)ng/L,Z=-6.255,P<0.001].FGF-21水平与运动、认知和感官功能呈正相关.在校正人口学特征、生活行为和共病状态后,相对于低水平FGF-21组,中水平组和高水平组老人中出现IC下降的OR值分别为0.58(95%CI:0.36~0.94,P=0.029)和 0.41(95%CI:0.24~0.68,P=0.001).ROC 曲线下面积为 0.644(95%CI:0.602~0.691).结论 血浆FGF-21与老年人IC呈正相关,对IC下降可能具有识别价值,可作为潜在的生物标志物和干预靶点.

Objective To examine the association between plasma fibroblast growth factor(FGF)-21 levels and intrinsic capacity(IC)among community-dwelling older adults.Methods A cross-sectional survey was conducted from February to August 2023 in four communities in Beijing.We enrolled 671 residents aged≥60 years and collected information on demographics,physical examinations,and plasma samples.Intrinsic capacity was evaluated according to the 2015 WHO framework across five domainsand classified as normal IC(9-10 points)or declined IC(0-8 points).Plasma FGF-21 concentrations were quantified by enzyme-linked immunosorbent assay and categorized into low,medium,and high tertiles.Spearman correlation coefficients were calculated to assess the relationship between FGF-21 levels and IC scores.Multivariable logistic regression and Receiver operating characteristic(ROC)curve analyses were employed to determine the impact and discriminative ability of FGF-21 for identifying IC decline.Results Among the participants,249(37.11%)exhibited IC decline.FGF-21 levels were significantly higher in the normal-IC group than in the declined-IC group[3.23(2.78,3.88)ng/L vs 2.81(2.48,3.31)ng/L;Z=-6.255,P<0.001].FGF-21 concentrations were positively correlated with locomotion,cognition,and sensory function.After adjustment for demographic characteristics,lifestyle behaviors,and comorbidities,the odds ratios for IC decline in the medium and high FGF-21 tertiles versus the low tertile were 0.58(95%CI:0.36-0.94,P=0.029)and 0.41(95%CI:0.24-0.68,P=0.001),respectively.The area under the ROC curve was 0.644(95%CI:0.602-0.691).Conclusions Higher plasma FGF-21 levels are positively associated with intrinsic capacity in older adults and may have identification value for IC decline,highlighting its potential as a biomarker and therapeutic target.
