MRI对儿童及青少年肘关节韧带损伤的诊断价值及影像学表现OA
Diagnostic value andand imaging findings of MRI for elbow ligament injuries in children and adolescents
目的 探讨儿童及青少年肘关节韧带损伤的磁共振成像(MRI)表现,分析MRI对该病的诊断价值.方法 选择2022年1月—2024年1月收治的90例肘关节损伤儿童及青少年患者作为研究对象.所有患者均接受高频超声和MRI检测.以随访及手术结果为金标准,对比高频超声和MRI检查对肘关节韧带损伤诊断结果的一致性,记录MRI影像学特征,使用受试者工作特征(ROC)曲线分析二者的诊断效能.对比不同检查方式对患儿肘关节韧带损伤程度的检出率.结果 经随访及手术证实确诊的90例患者中67例(74.44%)为肘关节韧带损伤、14例(15.56%)为肌腱损伤、9例(10.00%)为肘关节骨折.肘关节韧带损伤的MRI表现为T1加权成像序列可见拉伤的韧带模糊增粗,纤维部分连续,质子密度加权脂肪抑制序列表现为不均匀增高信号;撕裂韧带表现为纤维结构模糊,纤维增粗、不连续、局部断裂,原条形低信号区域呈高信号改变;完全撕裂韧带表现为纤维回缩中断、断裂端韧带结构消失,并伴有周围组织水肿.高频超声对肘关节韧带损伤检查结果的Kappa值为0.580,MRI对肘关节韧带损伤的Kappa值为0.733.经ROC曲线分析,高频超声、MRI诊断肘关节韧带损伤的曲线下面积分别为0.802、0.861,敏感度分别为86.57%、73.91%,特异度分别为94.03%、78.26%.高频超声和MRI对肘关节韧带损伤程度的检出率比较差异无统计学意义(P>0.05).结论 MRI检查对儿童及青少年肘关节韧带损伤的诊断价值较高,可获得典型影像学图像结果,能为临床诊断提供可靠的参考.
Objective To investigate the magnetic resonance imaging(MRI)findings of elbow ligament injuries in children and adolescents,and to assess the diagnostic value of MRI for this condition.Methods A total of 90 children and adolescents with elbow joint injuries who were admitted from January 2022 to January 2024 were selected as the study subjects.All patients underwent high-frequency ultrasound and MRI examinations.With the follow-up and surgical results as the gold standard,the consistency of the diagnostic results of high-frequency ultrasound and MRI for elbow ligament injuries was compared.The MRI imaging features were recorded,and the diagnostic efficacy of the two methods was analyzed using the receiver operating characteristic(ROC)curve.The detection rates of different examination methods for the severity of elbow ligament injuries in children were compared.Results Among the 90 patients who were confirmed through follow-up and surgery,67 patients(74.44%)were diagnosed with ligament injuries of the elbow joint,14 patients(15.56%)had tendon injuries,and 9 patients(10.00%)had fractures of the elbow joint.The MRI findings of ligament injuries of the elbow joint showed that on T1-weighted imaging(T1WI),the injured ligaments appeared blurred and thickened,with partial continuity of the fibers.On proton density-weighted fat-suppressed(PDWI-FS)sequences,it presented as heterogeneous hyperintensity.The torn ligaments presented as blurred fibrous structure,with fibers that were thickened,discontinuous,and local fractures,and the original band-like low-signal areas showed high-signal changes.Complete torn ligaments presented as retraction and disruption of the fibers,loss of ligamentous structure at the torn ends,and the presence of surrounding tissue edema.The Kappa value of high-frequency ultrasound for detecting elbow ligament injuries was 0.580,and the Kappa value of MRI for elbow ligament injuries was 0.733.Through ROC curve analysis,the area under the ROC curve of high-frequency ultrasound and MRI for diagnosing elbow ligament injuries was 0.802 and 0.861 respectively,with sensitivities of 86.57% and 73.91%,and specificities of 94.03% and 78.26%;there was no significant difference in the detection rate of the severity of elbow ligament injuries between high-frequency ultrasound and MRI(P>0.05).Conclusion MRI has a high diagnostic value for elbow ligament injuries in children and adolescents.It can provide typical imaging findings and offer reliable references for clinical diagnosis.
