目的 基于头颅磁共振成像分类系统(magnetic resonance imaging classification system,MRICS),探究痉挛型脑瘫(spastic cerebral palsy,SCP)患儿磁共振成像(magnetic resonance imaging,MRI)表现与其运动功能发育水平之间的关系.方法 回顾性纳入 122 名SCP患儿的资料,包括基于MRICS的颅脑MRI分型资料,以及粗大运动功能分级系统(gross motor function classification system,GMFCS)、粗大运动功能测试量表(gross motor function measure,GMFM)、手功能分级系统(manual ability classification system,MACS)、Peabody运动发育量表第2 版(Peabody developmental motor scale,2nd edition,PDMS-2)测评数据.运用多重线性回归模型分析MRICS下MRI分型与运动功能发育水平的关联.结果 85.25%的SCP患儿颅脑MRI表现存在异常,以脑白质损伤为主型最为常见(40.16%),且该类型患儿中低出生体质量、早产、出生窒息史的比例更高.SCP患儿MRICS下MRI分型与GMFM评分有关,与PDMS-2评分无显著关联.混合型患儿的GMFM评分显著低于其他类型患儿(P<0.05);在坐位和爬与跪功能区,脑白质损伤为主型和MRI正常型患儿的GMFM评分均高于其他类型的患儿(P<0.05).结论 大多数SCP患儿颅脑MRI存在异常,脑损伤模式与低出生体质量、早产、出生窒息史相关.在基于MRICS的颅脑MRI分型中,混合型SCP患儿粗大运动功能障碍最严重,脑白质损伤为主型和MRI正常型患儿粗大运动功能预后较好.

Objective To explore the correlation between magnetic resonance imaging(MRI)findings and motor function development level in children with spastic cerebral palsy(SCP)based on the magnetic resonance imaging classification system(MRICS).Methods Clinical data of 122 children with SCP were retrospectively collected,including cranial MRI classification results based on MRICS and assessment data from the gross motor function classification system(GMFCS),gross motor function measure(GMFM),manual ability classification system(MACS),and the Peabody developmental motor scales,2nd edition(PDMS-2).Multiple linear regression model was used to analyze the association between MRI classification under MRICS and motor function development level.Results Abnormal cranial MRI findings were observed in 85.25%of the children with SCP,with white matter injury-dominant type being the most common(40.16%).The proportions of children with low birth weight,premature birth,and a history of birth asphyxia were higher in this type.MRI classification under MRICS was correlated with GMFM scores in children with SCP,but showed no significant correlation with PDMS-2 scores.The GMFM scores of children with mixed-type SCP were significantly lower than those of children with other types(P<0.05).In the sitting and crawling-kneeling functional domains,the GMFM scores of children with the white matter injury-dominant type and normal MRI type were significantly higher than those of children with other types(P<0.05).Conclusion Most children with SCP have abnormal cranial MRI findings,and the pattern of brain injury is associated with low birth weight,premature birth,and a history of birth asphyxia.Among the cranial MRI classifications based on MRICS,children with mixed-type SCP have the most severe gross motor dysfunction,while those with white matter injury-dominant type and normal MRI type have a better prognosis of gross motor function.
