复方四黄外洗剂联合封闭负压间歇灌洗技术治疗糖尿病足感染伤口的疗效观察OA
Observation on Efficacy of Compound Sihuang External Lotion Combined with Closed Negative Pressure Intermittent Lavage Technique in Treatment of Infected Wounds of Diabetic Feet
目的 观察复方四黄外洗剂联合封闭负压间歇灌洗技术治疗糖尿病足感染伤口的临床疗效.方法 选取 2023 年 6 月—2024 年 9 月期间于中山市中医院接受治疗的糖尿病足感染伤口患者 128 例,基于随机数字表法分为观察组(64 例)和对照组(64 例).对照组予以盐水负压灌洗治疗,观察组予以复方四黄外洗剂联合封闭负压间歇灌洗治疗,经相同时间治疗后对比两组患者的临床疗效、实验室检验指标[降钙素原(PCT)、白细胞介素-6(IL-6)、C-反应蛋白(CRP)、白细胞(WBC)]、糖尿病足局部症状积分、中医证候评分.结果 观察组临床疗效远高于对照组(P＜0.05);治疗前,两组实验室检验指标、糖尿病足局部症状积分及中医证候评分差异无统计学意义(P＞0.05),治疗 2 周后与 4 周后,观察组PCT、IL-6、CRP、WBC水平改善优于对照组(P＜0.05),且糖尿病足局部症状与中医证候评分明显低于对照组(P＜0.05).结论 复方四黄外洗剂联合封闭负压间歇灌洗技术治疗糖尿病足感染伤口的临床疗效显著优于生理盐水负压灌洗治疗,能够有效改善实验室检验指标和糖尿病足局部症状.
Objective This paper aims to observe the clinical efficacy of Compound Sihuang External Lotion combined with the closed negative pressure intermittent lavage technique in the treatment of infected wounds of diabetic feet.Methods A total of 128 patients with infected wounds of diabetic feet who were treated in Zhongshan Traditional Chinese Medicine Hospital from June 2023 to September 2024 were selected and divided into the observation group(64 cases)and the control group(64 cases)based on the random number table method.The control group was treated with negative pressure saline lavage,and the observation group was treated with Compound Sihuang External Lotion combined with closed negative pressure intermittent lavage.After the same treatment time,the clinical efficacy,laboratory test indicators(procalcitonin(PCT),interleukin-6(IL-6),C-reactive protein(CRP),and white blood cell(WBC)),local symptom scores of diabetic feet,and traditional Chinese medicine syndrome score of the two groups were compared.Results The clinical efficacy of the observation group was significantly better than that of the control group(P<0.05).Before treatment,there was no statistically significant difference between the two groups in laboratory test indicators,local symptom scores of diabetic feet,and traditional Chinese medicine syndrome scores(P>0.05).After two weeks and four weeks of treatment,the observation group showed greater improvements in PCT,IL-6,CRP,and WBC levels compared to the control group(P<0.05),and the local symptoms of diabetic feet and traditional Chinese medicine syndrome scores were significantly lower in the observation group than those in the control group(P<0.05).Conclusion The clinical efficacy of Compound Sihuang External Lotion combined with the closed negative pressure intermittent lavage technique in the treatment of infected wounds of diabetic feet is significantly better than that of negative pressure saline lavage treatment,which can effectively improve laboratory test indicators and local symptoms of diabetic feet.
