针刺结合苗医弩药溻渍疗法治疗腰椎间盘突出症的临床观察OA
Clinical Observation on the Treatment of Lumbar Disc Herniation with Acupuncture Combined with External Application of Miao Medicine Crossbow Herb
目的 探讨针刺结合苗医弩药溻渍疗法治疗腰椎间盘突出症(LDH)的临床疗效和安全性.方法 本研究共纳入 2023 年 1 月—2024 年 7 月于贵州省习水县中医医院、贵州中医药大学第二附属医院针灸康复科就诊的患者 64 例,采用随机分组方式分为对照组(32 例,剔除 1 例、脱落 1 例)和治疗组(32 例,脱落 2 例).对照组给予常规针刺治疗,治疗组予常规针刺结合苗医弩药溻渍疗法,两组疗程均为 2 周.比较两组患者治疗前后的主观量表评分[视觉模拟评分法(VAS)评分、Oswestry功能障碍指数(ODI)、改良日本骨科学会腰痛评分(M-JOA)]、血清疼痛介质[血清β-内啡肽(β-EP)、P物质(SP)含量]水平及临床总有效率.结果 经 2周干预后,两组患者VAS评分、ODI、M-JOA及血清SP含量均较基线期降低(P＜0.01),血清β-EP含量较基线期升高(P＜0.05),且治疗组VAS评分、ODI、M-JOA、血清SP水平的下降幅度和临床总有效率均优于对照组(P＜0.05).结论 针刺结合苗医弩药溻渍疗法能够通过调节血清疼痛介质水平,有效改善LDH患者的疼痛程度、腰椎功能和日常生活能力,且安全性良好.
Objective To explore the clinical efficacy and safety of acupuncture combined with external application of Miao medicine crossbow herb on patients with lumbar disc herniation.Methods A total of 64 patients who were admitted to the Department of Acupuncture and Rehabilitation,Xishui County Hospital of Traditional Chinese Medicine and the Second Affiliated Hospital of Guizhou University of Traditional Chinese Medicine,from January 2023 to July 2024 were enrolled in this study and randomly divided into the treatment group(32 cases;2 cases dropped out)and control group(32 cases;1 case excluded,1 case dropped out).The control group was given conventional acupuncture treatment,while the treatment group was given conventional acupuncture combined with Miao medicine external application therapy.The treatment lasted for two weeks.Subjective scale scores[the visual analogue scale(VAS),Oswestry Disability Index(ODI),and modified Japanese Orthopaedic Association(M-JOA)score],expression of pain mediators[β-endorphin(β-EP)and substance P(SP)]and total clinical effective rate were compared between the two groups before and after treatment.Results After two weeks of intervention,VAS score,ODI index,M-JOA score and serum SP content in both groups were lower than before intervention(P<0.01),and serum β-EP content was higher than before intervention(P<0.05).The reductions in VAS score,ODI index,M-JOA score,and serum SP content,as well as the total clinical effective rate,were all significantly greaters in the treatment group than in the control group(P<0.05).Conclusion Acupuncture combined with external application of Miao medicine crossbow herb can effectively alleviate pain severity and improve lumbar function and daily living ability of LDH patients by adjusting the level of serum pain mediators,and it is safe.
