布比卡因和罗哌卡因对脐静脉造血干细胞的功能影响OA
Effects of bupivacaine and ropivacaine on the functional properties of human umbilical vein-derived hematopoi-etic stem cells
目的 探讨临床常用局部麻醉药布比卡因和罗哌卡因对新生儿脐带血CD34+CD133+造血干细胞生物学功能的影响,为干细胞采集及储存过程中麻醉药使用的安全性评估提供实验依据.方法 采集足月顺产新生儿脐带血,通过Ficoll密度梯度离心和免疫磁珠分选技术获得高纯度CD34+CD133+造血干细胞.分别给予不同浓度(0.1、0.5、1 mmol/L)的布比卡因和罗哌卡因处理,采用CCK-8法检测细胞活性,An-nexin V-FITC/PI双染流式细胞术检测细胞凋亡,荧光探针法联合流式细胞术检测细胞内一氧化氮(NO)水平,甲基纤维素集落培养分析造血潜能,Transwell实验评价细胞迁移能力.结果 与对照组比较,布比卡因对造血干细胞的细胞活性(P=0.027)、凋亡(P=0.008)、NO水平(P=0.002)、集落形成及迁移能力(P=0.006)均表现出显著的剂量依赖性抑制作用,在高浓度下尤其明显.罗哌卡因在多数浓度下对细胞功能影响较小,除在高浓度时对部分指标产生轻度抑制外,未表现出明确的剂量依赖关系,整体毒性作用相对温和.凋亡及NO结果提示,布比卡因可能通过诱导氧化应激促进细胞凋亡.结论 布比卡因对新生儿脐带血CD34+CD133+造血干细胞具有剂量依赖性毒性,而罗哌卡因在本实验浓度范围内相对安全.在分娩麻醉过程中,临床应优先选择罗哌卡因等毒性较低的局麻药,以保障脐带血干细胞的质量与后续应用价值.
Objective To investigate the effects of the commonly used local anesthetics bupivacaine and ropiva-caine on the biological functions of neonatal umbilical cord blood CD34+CD133+hematopoietic stem cells(HSCs),and to provide experimental evidence for evaluating the safety of anesthetic use during stem cell collection and storage.Meth-ods Umbilical cord blood samples were collected from full-term neonates delivered vaginally.CD34+CD133+HSCs were isolated using Ficoll density gradient centrifugation followed by immunomagnetic bead sorting.Cells were exposed to different concentrations(0.1,0.5,and 1 mmol/L)of bupivacaine or ropivacaine.Cell viability was assessed using the CCK-8 assay.Apoptosis was evaluated by Annexin V-FITC/PI double staining with flow cytometry.Intracellular nitric oxide(NO)levels were measured using fluorescent probes combined with flow cytometry.Hematopoietic potential was an-alyzed by methylcellulose colony-forming assays,and cell migratory capacity was evaluated using Transwell migration as-says.Results Compared with the control group.Bupivacaine significantly inhibited cell viability(P=0.027),colony formation,and migratory ability(P=0.006),and increased apoptosis(P=0.008)and intracellular NO levels(P=0.002)in a dose-dependent manner,with particularly pronounced effects at higher concentrations.In contrast,ropiva-caine exerted minimal effects on HSC function at most tested concentrations,showing only mild inhibitory effects on select-ed parameters at the highest concentration and no clear dose-response relationship overall.Apoptosis and NO results sug-gested that bupivacaine may promote HSC apoptosis through the induction of oxidative stress.Conclusion Bupivacaine exhibits dose-dependent cytotoxic effects on neonatal umbilical cord blood CD34+CD133+hematopoietic stem cells,whereas ropivacaine appears relatively safe within the concentration range tested.During obstetric anesthesia,ropivacaine or other local anesthetics with lower cytotoxicity should be preferentially selected to preserve the quality and clinical utility of umbilical cord blood stem cells.
