直肠癌癌结节影像学评估现状与挑战:从解剖形态到人工智能OA
Current status and challenges in imaging evaluation of tumor deposits in rectal cancer:from anatomical morphology to artificial intelligence
直肠癌癌结节是独立于淋巴结转移的高危病理特征,在直肠癌风险分层及治疗决策中具有重要价值,被纳入直肠癌磁共振影像评估标准,且定量功能成像及人工智能融合模型也对癌结节诊断效能有提升作用.直肠癌影像评估体系正由单纯的形态学描述向涵盖功能成像、人工智能及生物学行为预测的综合模式转变.本文重点剖析高阶成像技术的临床转化壁垒、病理"金标准"偏差对人工智能模型的影响以及单中心数据的泛化局限,以期为临床诊疗提供客观参考.
Tumor deposits(TDs)in rectal cancer represent a high-risk pathological feature independent of lymph node metastasis.They hold significant value in risk stratification and treatment decision-making for rectal cancer and have been incorporated into magnetic resonance imaging-based rectal cancer assessment criteria.Furthermore,quantitative func-tional imaging and integrated artificial intelligence(AI)models show potential in enhancing the diagnostic efficacy for TDs.The imaging evaluation system for rectal cancer is evolving from purely morphological descriptions toward a compre-hensive model that encompasses functional imaging,AI,and predictions of biological behavior.This article focuses on ana-lyzing the barriers to the clinical translation of advanced imaging technologies,the impact of deviations from the patho-logical"gold standard"on AI models,and the generalizability limitations of single-center data,aiming to provide an ob-jective reference for clinical diagnosis and treatment.
