肿瘤相关性抑郁(cancer-related depression,CRD)是在肿瘤诊断和治疗期间出现的病理性情绪状态.微生物-肠-脑轴(microbiome-gut-brain axis,MGBA)揭示了大脑、肠道和肠道微生物群之间复杂的双向通信网络,肠道微生物已成为抑郁症的重要病理标志,亦是治疗MGBA相关疾病的新靶点."上下交损,当治其中"针对"上损及下,下损及上"的复杂病机,强调中焦为升降之枢."上下交损"是郁证病机关键,CRD呈现上下交损、多脏同病的复杂病理特点,且以肝、脾、心三脏为核心.CRD患者"当治其中",以"调中焦"为要,通过疏通中焦、恢复气机升降之枢."治中"之法在CRD治疗中的具体应用包括:疏肝解郁、枢机畅通,化痰开郁、运化有权,补益心脾、脑有所养.并结合临床验案阐述"上下交损,当治其中"在CRD中的具体应用.
Cancer-related depression(CRD)refers to a pathological emotional state that emerges during cancer diagnosis and treatment.The"microbiome-gut-brain axis"(MGBA)reveals a complex bidirectional network connecting the brain,gut,and gut microbiota.Gut microbiota has become a key pathological marker of depression and a novel target for treating MGBA-related disorders.The principle of"treating the middle when both upper and lower impaired"addresses complex pathogenesis involving mutual impairment between upper and lower body regions,emphasizing the middle jiao as the pivot of qi movement."Mutual impairment of the upper and lower"is central to the pathogenesis of depression,and CRD exhibits such complexity with multi-organ involvement,particularly the liver,spleen,and heart.For CRD patients,the principle advocates"regulating the middle jiao"as the core approach—unblocking the middle region and restoring the qi movement pivot.Clinical applications include soothing the liver and relieving depression,smoothing the pivot,resolving phlegm and relieving depression,enhancing the function of transportation and transformation,and nourishing the heart,spleen and brain.A clinical case is included to illustrate the application of this principle in CRD.
