黄精降压汤联合氨氯地平治疗高血压病患者合并焦虑状态的效果观察OA
Clinical efficacy of Huangjing Jiangya Decoction combined with amlodipine in improving hypertensive patients with anxiety
目的 观察黄精降压汤联合氨氯地平治疗高血压病患者合并焦虑状态的效果.方法 选择2022年5月—2023年11月北京市第六医院中医科、心内科及神经内科门诊收治的气虚型原发性高血压合并焦虑状态患者71例,随机分为观察组、对照组.对照组给予西医常规治疗,口服苯磺酸氨氯地平片5 mg,1次/d;观察组在对照组基础上给予自拟黄精降压汤口服.2组均连续治疗8周.观察比较2组临床疗效及治疗前后诊室血压、家庭血压、动态血压、广泛性焦虑量表(GAD-7)评分、中医证候积分情况.结果 治疗后观察组临床疗效总有效率、中医证候总有效率高于对照组(均P<0.01).治疗后,观察组诊室血压、家庭血压均较治疗前降低(P<0.05),且均低于对照组(P<0.01);观察组24 h SBP、DBP较治疗前降低(P<0.05),且均低于对照组(P<0.01);观察组GAD-7评分较治疗前降低(P<0.05),且低于对照组(P<0.01);观察组中医证候积分较治疗前降低(P<0.05),且低于对照组(P<0.01).2组治疗过程中均未见恶心、呕吐等不良反应,肝、肾功能指标等均未见异常.结论 黄精降压汤联合氨氯地平可明显降低气虚型高血压病合并焦虑状态患者的血压,改善其焦虑状态和临床症状,整体疗效优于单纯氨氯地平治疗,且安全性高.
Objective To observe the clinical efficacy of Huangjing Jiangya Decoction combined with amlodipine in treating patients with hypertension complicated by anxiety.Methods A total of 71 patients with primary hypertension of qi-deficiency type and comorbid anxiety,treated at the Traditional Chinese Medicine,Cardiology,and Neurology Outpatient Departments of Beijing Sixth Hospital from May 2022 to November 2023,were randomly divided into an observation group and a control group.The control group received conventional Western medicine treatment with amlodipine besylate tablets 5 mg once daily.The observation group received additional oral administration of self-formulated Huangjing Jiangya Decoction on top of the control regimen.Both groups were treated continuously for 8 weeks.Clinical efficacy,clinic blood pressure,home blood pressure,24-hour ambulatory blood pressure,GAD-7 score,and TCM syndrome scores before and after treatment were compared between the two groups.Results After treatment,the total clinical efficacy and total TCM syndrome efficacy in the observation group were higher than those in the control group(both P<0.01).Clinic and home blood pressure in the observation group were significantly reduced compared to baseline(both P<0.05)and were lower than those in the control group(both P<0.01).The 24-hour systolic and diastolic blood pressure in the observation group decreased significantly compared to baseline(both P<0.05)and were lower than those in the control group(both P<0.01).The GAD-7 score and TCM syndrome scores in the observation group were significantly lower than before treatment(both P<0.05)and lower than in the control group(both P<0.01).No adverse reactions such as nausea or vomiting were observed during treatment,and liver and kidney function indicators remained normal in both groups.Conclusion Huangjing Jiangya Decoction combined with amlodipine can significantly reduce blood pressure,improve anxiety symptoms and clinical manifestations in patients with qi-deficiency hypertension and anxiety,with overall efficacy superior to amlodipine alone,and demonstrates high safety.
