目的:探究与分析血清25-羟基维生素D3[25-(OH)D3]、甲壳质酶蛋白-40(YKL40)、N末端B型钠尿肽前体(NT-proBNP)、可溶性生长刺激表达基因2蛋白(sST2)对川崎病患儿冠状动脉病变风险的预测价值.方法:回顾性分析2022年10月—2024年9月苏州市吴江区儿童医院收治的42例川崎病患儿(将其设为研究组)以及同时期进行常规体检的45名健康儿童(将其设为对照组)的临床资料.根据研究组心脏彩超结果,又分为冠状动脉病变组(18例)及无冠状动脉病变组(24例).比较研究组和对照组血清指标,预测冠状动脉病变风险的价值.结果:研究组血清25-(OH)D3水平低于对照组,YKL40、NT-proBNP、sST2水平均高于对照组,差异有统计学意(P<0.05).冠状动脉病变组血清YKL40、NT-proBNP、sST2水平高于无冠状动脉病变组,25-(OH)D3水平低于无冠状动脉病变组,差异有统计学意(P<0.05).研究组血清25-(OH)D3水平与冠状动脉病变呈负相关,差异有统计学意(P<0.05).YKL40、NT-proBNP、sST2 水平与冠状动脉病变呈正相关(P<0.05).25-(OH)D3、YKL40、NT-proBNP、sST2 预测冠状动脉病变风险的AUC分别为0.921、0.931、0.745、0.963.结论:血清25-(OH)D3、YKL40、NT-proBNP、sST2水平与KD患儿冠状动脉病变密切相关,联合检测有助于临床早期识别KD患儿冠状动脉病变的高危人群.

Objective:To explore and analyze the predictive value of serum 25-hydroxy vitamin D3[25-(OH)D3],human chitinase protein-40(YKL40),N-terminal pro-B-type natriuretic peptide(NT-proBNP),and Soluble growth stimulation expressed gene 2(sST2)for the risk of coronary artery lesions in children with Kawasaki disease.Method:Clinical data of 42 children with Kawasaki disease(designated as the study group)admitted to Suzhou Wujiang Children's Hospital between October 2022 and September 2024,and 45 healthy children(designated as the control group)who underwent routine physical examinations during the same period,were retrospectively analyzed.Based on the echocardiography results of the study group,they were further divided into a coronary artery lesion group(18 cases)and a non-coronary artery lesion group(24 cases).Serum markers were compared between the study group and the control group to assess their value in predicting the risk of coronary artery lesions.Result:Serum 25-(OH)D3 levels in the study group were lower than those in the control group,while YKL-40,NT-proBNP,and sST2 levels were significantly higher(P<0.05).Within the study group,the coronary artery lesion subgroup exhibited higher serum levels of YKL-40,NT-proBNP,and sST2,and lower levels of 25-(OH)D3 compared to the non-coronary artery lesion subgroup(P<0.05).Serum 25-(OH)D3 level showed a negative correlation with the presence of coronary artery lesions(P<0.05),whereas YKL-40,NT-proBNP,and sST2 levels showed positive correlations(P<0.05).The areas under the curve(AUCs)for predicting coronary artery lesion risk were 0.921 for 25-(OH)D3,0.931 for YKL-40,0.745 for NT-proBNP,and 0.963 for sST2.Conclusion:The levels of serum 25-(OH)D3,YKL40,NT-proBNP,and sST2 are closely related to coronary artery lesions in children with KD.Combined detection is helpful for the early clinical identification of high-risk populations of coronary artery lesions in children with KD.
