目的:探讨CT灌注(CTP)与MRI弥散加权成像(DWI)联合应用对缺血性脑卒中的评估价值.方法:选取 2021 年12 月—2024 年 12 月白银市中西医结合医院收治的 120 例缺血性脑卒中患者,均行CTP与MRI DWI检查.分析两者对缺血性脑卒中的诊断效能、评估缺血半暗带情况以及与患者临床预后的相关性.结果:CTP联合MRI DWI对缺血性脑卒中的诊断效能明显优于任一单独检查,其敏感度与特异度均更高,且联合诊断的AUC达 0.850,差异均具有统计学意义(P<0.05).此外,联合应用能更准确地评估缺血半暗带,其体积与患者 90 d改良Rankin量表评分呈正相关(r=0.678,P<0.001).结论:CTP与MRI DWI联合可从血流灌注和水分子扩散角度全面评估缺血性脑卒中,为临床治疗和预后判断提供重要依据.

Objective:To investigate the clinical value of combining computed tomography perfusion(CTP)and diffusion-weighted imaging(DWI)in the evaluation of ischemic stroke.Method:A total of 120 patients with ischemic stroke admitted to Baiyin Hospital of Integrated Traditional Chinese and Western Medicine from December 2021 to December 2024 were enrolled.All patients underwent both CTP and MRI DWI examinations.The diagnostic efficacy of the two modalities for ischemic stroke,the assessment of the ischemic penumbra,and the correlation with clinical outcomes were analyzed.Result:The diagnostic efficacy of combining CTP with MRI DWI for ischemic stroke was significantly superior to either examination alone,demonstrating higher sensitivity and specificity.The AUC of the combined diagnosis reached 0.850,with statistically significant differences(P<0.05).Furthermore,the combined application allowed more accurate assessment of the ischemic penumbra,whose volume showed a significant positive correlation with the 90-day modified Rankin Scale score(r=0.678,P<0.001).Conclusion:The combination of CTP and MRI DWI provides a comprehensive evaluation of ischemic stroke from both hemodynamic and molecular diffusion perspectives,offering critical insights for clinical management and prognostic assessment.
