消渴肾病是临床常见疾病,深层次地理清消渴肾病病机对本病的治疗至关重要.本篇将基于《灵枢》"脉胀"理论,即"营气循脉,卫气逆为脉胀""真邪相攻,两气相搏,乃合为胀也",从营卫的角度对消渴肾病发病的病理过程进行探讨.提出了"络-经-冲"脉络网的概念,阐明了脉络网是毒邪导致肾络损伤的生理基础,营卫失调所发之"脉胀",是毒损肾络的病理基础,论证了"毒损肾络"之"损"是"脉胀"所致,并介绍了营卫在"络-经-冲"脉络网不同阶段的影响.进一步完善了消渴肾病病机,供临证施治参考.

Diabetic nephropathy is a common clinical disease,and a thorough understanding of its pathogenesis is crucial for effective treatment.Based on the"vessel distension"theory from the Spiritual Pivot,which states that"nutrient Qi circulates through the vessels,while defensive Qi counterflows,leading to vessel distension"and"when genuine Qi and pathogenic factors contend,the two forces clash,resulting in distension",this paper explores the pathological process of diabetic nephropathy from the perspective of nutrient Qi and defensive Qi.The concept of the"collateral-channel-thoroughfare vessel network"is proposed,elucidating that this network serves as the physiological basis for toxin-induced damage to the kidney collaterals.The"vessel distension"caused by the imbalance of nutrient Qi and defensive Qi is identified as the pathological basis for toxin-induced damage to the kidney collaterals.It is argued that the"damage"in"toxin damaging the kidney collaterals"is a consequence of"vessel distension".Additionally,the influence of nutrient Qi and defensive Qi at different stages of the"collateral-channel-thoroughfare vessel network"is discussed.This study further refines the pathogenesis of diabetic nephropathy,providing valuable insights for clinical diagnosis and treatment.
