水光针注射致面部肉芽肿3例及文献复习OA
Report and literature review of three cases of facial granuloma after mesotherapy injection
目的 探讨水光针注射后肉芽肿的临床特点、治疗方案及疗效.方法 报道2023年至2024年华中科技大学同济医学院附属协和医院皮肤科门诊就诊的 3 例水光针注射导致的肉芽肿患者,采用联合治疗方案,系统采用口服米诺环素抗炎,局部行二氧化碳点阵激光联合曲安奈德湿敷,结节消退后改为强脉冲光及化学换肤术治疗色沉及毛细血管扩张.结果 3 例患者在门诊治疗后,皮损均好转,面部丘疹消退变平,余少许色沉斑,仍继续随访治疗改善色沉.结论 水光针的广泛使用也引起了不良反应的数量增多,尤其是面部的异物肉芽肿,除了系统使用药物抗炎之外,局部治疗也非常重要.采用二氧化碳点阵激光联合曲安奈德湿敷,相较于局部注射激素副作用更小,不易导致局部萎缩凹陷.结节消退后改为强脉冲光治疗色沉及毛细血管扩张,可有效进一步改善皮损.
Objective To investigate the clinical features,treatment approaches and therapeutic effects of granuloma after mesotherapy injection.Methods Three patients with granuloma caused by mesotherapy injection who visited the dermatology outpatient department of Union Hospital,Tongji Medical College,Huazhong University of Science and Technology from 2023 to 2024 were reported.A combined treatment plan was adopted,with oral minocycline as the systemic anti-inflammatory treatment,and carbon dioxide fractional laser combined with triamcinolone acetonide wet compress for local treatment.After the nodules subsided,intense pulsed light and chemical peelings were used to treat pigmentation and telangiectasia.Results After outpatient treatment,the skin lesions of the 3 patients all improved.The facial papules subsided and flattened,leaving a few pigmented spots.Follow-up treatment was still continued to improve the pigmentation.Conclusion The extensive use of mesotherapy injections has also led to an increase in the number of adverse reactions,especially foreign body granulomas on the face.In addition to systemic anti-inflammatory treatment with antibiotics,local treatment is also very important.We used carbon dioxide fractional laser combined with triamcinolone wet compress,which has fewer side effects than intralesional corticosteroids and is not easy to cause local atrophy.After the nodules regressed,intense pulsed light therapy for pigmentation and telangiectasia could effectively further improve the skin lesions.
