目的:探讨MRI与CT图像融合配准技术在中心型肺癌放疗肿瘤靶区勾画中的应用价值.方法:选取82例行放疗的中心型肺癌患者为研究对象,于放疗前分别利用CT图像及CT与MRI图像融合技术完成靶区界定.比较2种勾画方式大体肿瘤靶区(GTV)、临床靶区(CTV)的体积、大小(长度、厚度)、变异系数、形态一致性(最大值与最小值比值,即Ratio)、照射剂量、靶区边界内外SNR、图形灰度值等的差异,并分析CT与MRI图像融合前后靶区特征的变化.结果:融合图像的靶区最小和最大照射剂量均低于CT图像(均P<0.05);CT图像勾画的GTV和CTV体积、长度、厚度均大于融合图像(均P<0.05);CT图像GTV及CTV的变异系数、Ratio均大于融合图像(均P<0.05);CT图像的靶区边界内外SNR、图形灰度值均小于融合图像(均P<0.05).结论:仅依赖CT图像界定肿瘤放疗靶区存在精确度局限,MRI与CT图像融合技术则能显著提高中心型肺癌放疗靶区界定的精确度,有助于提高放疗的精准性.

Objective:To assess the effectiveness of MRI-CT image fusion registration for target volume definition in central lung cancer radiotherapy.Methods:A study was conducted on 82 patients with central lung cancer who recieved radiotherapy,and their tumor target volumes were delineated using both CT scans and MRI-CT image fusion.The size(length,thickness),coefficient of variation(CV),morphological consistency(ratio of maximum to minimum,Ratio),of the gross tumor volume(GTV)and clinical target area(CTV),radiation dose,SNR inside and outside the target area boundary,and graphic grayscale value were compared between the two delineation methods.And the changes in the characteristics of the target area before and after fusion were analyzed.Results:The minimum and maximum radiation doses in the fusion images were significantly lower than those in CT images(both P<0.05).The volume,length and thickness of GTV and CTV in CT images were all larger than those in the fusion images(all P<0.05).The CV and Ratio values of GTV and CTV in CT images were higher than those in the fusion images(all P<0.05).The SNR and graph gray value in CT images were lower than those in the fusion images(both P<0.05).Conclusions:The study concludes that relying on CT scans alone for defining tumor target volumes in radiotherapy is limited in precision.In contrast,the use of MRI-CT image fusion technology significantly enhances the accuracy of target volume delineation for central lung cancer,leading to improved precision in radiotherapy delivery.
