目的:探讨基于术前多参数MRI特征及定量参数构建的预测模型对乳腺癌前哨淋巴结(SLN)转移的预测价值.方法:回顾性分析经病理确诊的128例乳腺癌患者,术前均行多参数MRI扫描,包括T1WI、T2WI、DWI和动态对比增强MRI(DCE-MRI).根据病理结果将患者分为SLN转移组(68例)和SLN未转移组(60例).比较2组术前多参数MRI特征及定量参数[容量转运常数(Ktrans)、速率常数(Kep)、血管外间隙容积分数(Ve)及ADC值].利用多因素logistic回归分析进一步筛选差异有统计学意义的指标作为预测SLN转移的独立危险因素,构建预测模型,绘制预测模型的列线图.采用ROC曲线分析模型的预测效能,绘制校准曲线评价模型的稳定性,并通过决策曲线评估模型的临床应用价值.结果:2组Ktrans、Kep、肿瘤最大径、ADC值、瘤周血管征象、淋巴结脂肪门、淋巴结边缘及腋窝淋巴结(ALN)对称性差异均有统计学意义(均P<0.05).多因素logistic回归分析显示,较高的Ktrans值、瘤周水肿、瘤周血管征象及较低ADC值是预测SLN转移的独立危险因素(OR=2.372,4.282,8.264,0.042;均P<0.05).预测模型预测乳腺癌SLN转移的AUC为0.832,决策曲线分析显示预测模型的临床实用性较高.结论:基于术前MRI特征及定量参数(Ktrans、ADC值)构建的预测模型对乳腺癌SLN状态具有较高的预测价值,有助于优化乳腺癌患者的治疗策略.

Objective:To investigate the predictive value of a model constructed based on preoperative multi-parameter MRI features and quantitative parameters for sentinel lymph node(SLN)metastasis in breast cancer.Methods:128 cases of breast cancer diagnosed pathologically were retrospectively analyzed.All patients underwent multi-parameter MRI before surgery,including T1WI,T2WI,DWI and dynamic contrast enhanced MRI(DCE-MRI).The patients were divided into the metastasis group(68 cases)and the non-metastasis group(60 cases)according to pathological results.The preoperative multiparametric MRI features and quantitative parameters(Ktrans,Kep,Ve,and ADC value)between two groups were compared.Tthe indicators with significant statistical differences were further screened by multivariable logistic regression analysis,and a predictive model was constructed.ROC curve was used to analyze the prediction efficiency of the model,calibration curve was drawn to evaluate the calibration degree of the model,and decision curve analysis(DCA)was used to evaluate the clinical application value of the prediction model.Results:Significant differences were found in Ktrans,Kep,tumor size,ADC value,peritumoral vascular signs,lymph node adipose hilum,lymph node margin and ALN symmetry between the two groups(all P<0.05).Multivariable logistic regression analysis showed that the higher Ktrans,peritumoral edema,peritumoral vascular signs and the lower ADC value were independent risk factors for SLN metastasis(OR=2.372,4.282,8.264,0.042,all P<0.05).The AUC of the prediction model was 0.832,and DCA showed that the model had strong clinical practicability.Conclusions:The prediction model based on preoperative MRI features and quantitative parameters(Ktrans,ADC value)has a high predictive performance in evaluating the SLN status in breast cancer,which is conducive to optimizing the current treatment strategy for breast cancer patients.
