目的:分析Ki-67高表达胃肠道间质瘤(GIST)的CT特征影响因素.方法:收集191例GIST患者的临床及CT资料.191例分为高表达组(Ki-67>5%)71例和低表达组(Ki-67≤5%)120例.比较2组CT定性及定量特征.采用Spearman相关性分析评估Ki-67高表达与CT特征的相关性.采用二元logistic回归分析筛选Ki-67高表达GIST的独立影响因素.结果:2组病灶长径、短径、危险度分级、生长方式、囊变、门脉期强化程度和门脉期强化率的差异均有统计学意义(均P<0.05),且这些特征与Ki-67高表达呈弱相关(r值0.147~0.371).logistic回归分析显示,中/高危险度、长径(>5.85 cm)、短径(>2.95 cm)、腔内生长、囊变、门脉期强化程度(>58.25 HU)和门脉期强化率(>1.73)是Ki-67高表达GIST的独立影响因素.结论:中/高危险度、长径(>5.85 cm)、短径(>2.95 cm)、腔内型、囊变、门脉期强化程度(>58.25 HU)和门脉期强化率(>1.73)是Ki-67高表达GIST的独立影响因素.

Objective:To investigate CT features associated with high Ki-67 expression in gastrointestinal stromal tumor(GIST)and to identify potential imaging influencing factors.Methods:The clinical and CT data of 191 GIST patients were collected.All patients were stratified into a high expression group(71 cases,Ki-67>5%)and a low expression group(120 cases,Ki-67≤5%).Comparative analysis of CT qualitative and quantitative characteristics between the two groups was executed.Spearman correlation analysis was used to analyze the CT features related to the high Ki-67 expression.Binary logistic regression analysis was employed to ascertain the influencing factors associated with high Ki-67 expression in GIST.Results:Statistically significant differences were identified in tumor maximum and minimum diameters,risk grade,growth pattern,cystic degeneration,enhancement degree and rate in the portal venous phase between the two groups(all P<0.05).These CT features exhibited a weak correlation with high Ki-67 expression(r=0.147-0.371).Logistic regression analysis revealed that intermediate/high risk grade,maximum diameter(>5.85 cm),minimum diameter(>2.95 cm),intraluminal growth pattern,cystic degeneration,enhancement degree(>58.25 HU)and enhancement rate(>1.73)in the portal venous phase were the independent influencing factors for high Ki-67 expression in GIST.Conclusions:Intermediate/high risk grade,maximum diameter(>5.85 cm),minimum diameter(>2.95 cm),intraluminal growth pattern,cystic degeneration,enhancement degree(>58.25 HU)and enhancement rate(>1.73)in the portal venous phase are the independent influencing factors for high Ki-67 expression in GIST.
