基于医院信息化技术的医疗设备五维管理模型在超声诊断设备质量管理中的价值研究OA
Value of 5D management model based on hospital information technology for medical equipment in quality management for equipment of ultrasound diagnosis
目的:基于医院信息化技术构建医疗设备五维管理模型,探究其在超声诊断设备质量管理中的应用价值.方法:基于医院信息化技术、五维管理(战略、资源、价值、竞争和创新)和全面质量管理与精益管理理论,结合医疗设备管理特点,构建医疗设备五维管理模型.选取2022年1月至2024年10月武汉市第一医院临床在用的5台彩色多普勒超声诊断仪和入院就诊并接受其超声检查的106例患者,将2022年1月至2023年4月的设备使用期间的管理采用常规管理方法,2023年5月至2024年10月的设备使用期间的管理采用基于医院信息化技术的五维管理模型管理方法(五维模型管理方法),每种管理方法涉及超声检查的病例为53例患者,对比两种管理方法的患者超声诊断结果与临床符合情况、超声诊断设备管理质量评分、工作流程管理评分和缺陷事件发生情况.采用自制满意度量表对使用超声设备的医护人员和患者进行满意度评价.结果:采用五维模型管理方法的53例患者超声诊断图片质量良好率和临床诊断符合率分别为96.23%(51/53)和94.34%(50/53),均高于常规管理方法,差异均有统计学意义(x2=3.975、4.296,P<0.05);五维模型管理方法的超声诊断设备图像存储、设备功能、设备运行、软件管理和配置管理质量评分均高于常规管理方法,差异均有统计学意义(t=7.793、8.175、6.183、7.175、5.605,P<0.05);五维模型管理方法超声诊断设备工作流程中的检查秩序、开关机规范、诊断报告及时性、诊断报告规范性、操作规范和时间管理评分均高于常规管理方法,差异均有统计学意义(t=5.907、3.988、4.449、4.969、3.578、3.286,P<0.05);五维模型管理方法的超声诊断设备管理缺陷事件发生率低于常规管理方法,差异有统计学意义(x2=4.970,P<0.05),患者满意度高于常规管理方法,差异有统计学意义(x2=4.296,P<0.05).医护人员对两种管理方法的满意度差异无统计学意义(P>0.05).结论:基于医院信息化技术的医疗设备五维管理模型可有效提高超声诊断设备质量管理水平,降低设备管理缺陷事件发生率,提高医护人员和患者的设备使用满意度.
Objective:To construct five-dimensional(5D)management model based on information technique of hospital for medical equipment,and explore application value of that in quality management for equipment of ultrasound diagnosis.Methods:A 5D management model for medical equipment was constructed on the basis of information technique of hospital,5D management(Strategy,Resources,Value,Competition,and Innovation),and the theories of comprehensive management and lean management that combined with management characteristics of medical equipment.From January 2022 to October 2024,5 used color Doppler equipment of ultrasound diagnosis of Wuhan No.1 Hospital and 106 patients who admitted to the hospital and received ultrasound examination were selected.The conventional management method was adopted to manage equipment during January 2022 and April 2023,while the management method of 5D management model based on information technique of hospital(5D model management method)was adopted to manage equipment during May 2023 and October 2024.The number of the cases that involved ultrasound examination were 53 cases in each management method.The consistency between ultrasound diagnosis and clinical result,the scores of management quality of equipment of ultrasound diagnosis,the scores of management for workflow,and incidence of defect events between two kinds of management methods were compared.A self-made satisfaction scale was used to evaluate the satisfaction of medical staffs and patients,who used ultrasound equipment.Results:The good rate of image quality of ultrasound diagnosis,and consistency rate between ultrasound diagnosis and clinical diagnosis of 53 patients of adopting 5D model management method were respectively 96.23%(51/53)and 94.34%(50/53),all of which were higher than those of conventional management method(x2=3.975,4.296,P<0.05).The scores for qualities of image storage,equipment function,equipment operation,software management and configuration management of equipment of ultrasound diagnosis of 5D model management method were all higher than those of conventional management method(t=7.793,8.175,6.183,7.175,5.605,P<0.05).The scores of inspection order,specification of"power on-off",timeliness of diagnostic report,standardization of diagnostic report,operation specification and time management in the workflow of equipment of ultrasound diagnosis of 5D model management method were all higher than those of conventional management method,and the differences were significant(t=5.907,3.988,4.449,4.969,3.578,3.286,P<0.05).The incidence of defect events of equipment of ultrasound diagnosis of 5D model management method was significantly lower than that of conventional management method(x2=4.970,P<0.05).The satisfaction of patients of 5D model management method was significantly higher than that of conventional management method(x2=4.296,P<0.05),and there was not statistically significant difference in the satisfaction of medical staffs between the two management methods(P>0.05).Conclusion:The 5D management model based on information technique of hospital for medical equipment can effectively improve the level of quality management for equipment of ultrasound diagnosis,and reduce the incidence of defect events of equipment management,and enhance the satisfaction of medical staffs and patients for using equipment.
