基于失效模式与影响分析的麻醉机风险管理研究OA
Risk management based on FMEA for anesthesia machine
目的:运用失效模式与影响分析(FMEA)方法识别麻醉机潜在失效风险,评估风险等级并制定针对性管控措施,提升麻醉机临床应用的安全性与可靠性.方法:选取2022年至2024年首都医科大学宣武医院临床在用的14台Avance CS2型麻醉机的维修记录及相关运行数据,组建麻醉机风险管理评价专家小组,从严重度、发生度和可探测度3个维度对麻醉机运行中的15类失效模式进行风险量化评估,计算风险优先数(RPN)并确定措施优先度(AP),重点分析高风险失效模式的原因与影响,制定针对性的风险控制策略.结果:麻醉机15类失效模式中风箱皮囊风险最高(RPN=240),电池、气体测量板、钠石灰罐密封盖等部件AP等级为中等.主要失效模式包括电池、氧电池、顺磁氧单元、气体测量板、手控机控转换开关、风箱皮囊、钠石灰罐密封盖和钠石灰罐密封垫8项,失效原因主要源于部件老化、机械磨损和密封失效,可通过人员培训、建立部件储备、优化预防性维护等措施降低设备故障风险.结论:FMEA方法能够有效识别麻醉机高风险失效部件并评估其风险程度,通过风险优先排序合理分配资源,需重点关注高风险的失效模式,提高风险管理的效率和效果,为医疗设备风险管理提供依据.
Objective:To identify potential failure risks of anesthesia machines using Failure Mode and Effects Analysis(FMEA),and evaluate risk levels of them,so as to assess risk levels and formulate targeted control measures for enhancing safety and reliability of anesthesia machines in clinical application.Methods:Maintenance records and operational data of 14 Avance CS2 anesthesia machines in clinical use at Xuanwu Hospital,Capital Medical University from 2022 to 2024 were collected.A expert panel of evaluating risk management for anesthesia machine was established to conduct quantitative assessment for risks of 15 types of failure modes during operation of anesthesia machine from three dimensions:severity(S),occurrence(O),and detectability(D).Risk Priority Numbers(RPN)were calculated,and Action Priority(AP)levels were determined.The reasons and effects of failure modes with high-risk were specially analyzed,and targeted strategies for risk control were formulated.Results:In 15 types of failure modes of anesthesia machines,the risk of the bladder of the bellows was highest(RPN=240),and the AP grades of batteries,boards of measuring gas,and sealing cap of sodium lime canister,etc.were moderate.The main failure modes were 8 items included battery,oxygen cell,paramagnetic oxygen unit,board of measuring gas,conversion switch of manual and machine control,bladder of bellows,sealing cap of sodium lime canister,and sealing gasket of sodium lime canister.The reasons of failure mainly were component aging,mechanical wear,and seal failure.The risks of the faults of equipment can be reduced through personnel training,establishment of the system of reserving component,and optimization of preventive maintenance.Conclusion:The FMEA method can effectively identify failure components with high-risk of anesthesia machines,and assess risk levels of them,and rationally allocate resources through priority ranking of risk.The special attention should be paid on failure mode with high-risk,so as to improve the efficiency and effectiveness of risk management,and provide basis of risk management for medical equipment.
