Association between Serum 25-Hydroxyvitamin D3 Levels and Immune-Related Adverse Event Induced by Immune Checkpoint Inhibitors

目的 探讨血清 25-羟维生素D3[25-(OH)D3]水平与免疫检查点抑制剂(ICIs)免疫相关不良反应(irAE)发生风险与严重程度的相关性.方法 选取攀枝花市中心医院2022 年8 月至2025 年2 月收治的148 例接受ICIs治疗的晚期恶性肿瘤患者,检测治疗前患者血清 25-(OH)D3 水平,分析其与irAE发生率与严重程度的关联.结果 血清25-(OH)D3 缺乏组(<10 ng/ml)的irAE发生率显著高于正常组(P<0.05),25-(OH)D3 是irAE的独立保护因素(OR=0.91,95%CI:0.86~0.96,P=0.001),小板计数/淋巴细胞计数比值(PLR)是irAE的独立风险因素(OR=1.82,95%CI:1.10~3.01,P=0.019).结论 血清25-(OH)D3 水平是ICIs发生irAE的重要风险因素,适时监测25-(OH)D3 水平或补充维生素D3 可为irAE的预防提供新依据.

Objective To explore the correlation between serum 25-hydroxyvitamin D3[25-(OH)D3]levels and the risk and severity of immune-related adverse event(irAE)caused by immune checkpoint inhibitors(ICIs).Methods A total of 148 patients with advanced malignant tumors who received ICIs treatment and were admitted to Panzhihua Central Hospital from August 2022 to February 2025 were selected.The serum 25-(OH)D3 levels of the patients before treatment were detected,and the association between it and the incidence and severity of irAE was analyzed.Results The incidence of irAE in the serum 25-(OH)D3 deficiency group(<10 ng/ml)was significantly higher than that in the normal group(P<0.05),and 25-(OH)D3 was an independent protective factor for irAE(OR=0.91,95%CI:0.86~0.96,P=0.001),the ratio of small plate count to lymphocyte count(PLR)was an independent risk factor for irAE(OR=1.82,95%CI:1.10~3.01,P=0.019).Conclusion Serum 25-(OH)D3 level is an important risk factor for irAE in ICIs.Timely monitoring of 25-(OH)D3 level or supplementation of vitamin D3 may provide new evidence for the prevention of irAE.
