医用耗材全生命周期一体化智能供应链系统的构建研究OA

目的:构建医用耗材全生命周期一体化智能供应链系统,探讨数字技术在提升医院供应链管理效能中的应用价值.方法:系统采用数据层、网络层、共识层、智能合约层和应用层分层式技术架构,设置智能采购、动态库存和全程追溯3大功能模块构建医用耗材全生命周期一体化智能供应链系统.选取2023年1月至2024年12月重庆市3家综合型三级甲等医院的150 072台手术资料以及术中使用的医用耗材相关资料数据,将2023年1月至12月的74 316台手术资料设为医用耗材全生命周期一体化智能供应链系统应用前资料,2024年1月至12月的75 756台手术资料设为系统应用后资料,对比系统应用前后3家医院中的耗材领用平均耗时、库存周转天数、单台手术耗材消耗成本、计费差错率和账物相符率,以及系统应用后干预效果变化情况.结果:系统应用后3家医院合并效应的耗材领用平均耗时、库存周转天数、单台手术耗材消耗成本和平均计费差错率分别为(2.7±0.8)h、(6.8±1.5)d、(10 980.2±1210.7)元和(0.8±0.3)%,均低于应用前,差异有统计学意义(t=35.62、43.75、17.93、40.81,P<0.05);而平均账物相符率为(99.1±0.5)%,高于系统应用前,差异有统计学意义(t=29.47,P<0.05).3家医院干预效果的耗材领用平均耗时降幅、平均库存周转天数降幅、平均单台手术耗材消耗成本降幅、平均计费差错率降幅和账物相符率增幅比较,差异均有统计学意义(F=9.05、10.12、13.08、8.27、29.75,P<0.05),且存在较高的异质性(I2>50%).结论:医用耗材全生命周期一体化智能供应链系统应用,能够提升医用耗材领用效率,加快医用耗材库存周转流转,控制手术耗材成本,提高耗材计费精准度和账物相符水平.

Objective:To construct an integrated intelligent supply chain system for full lifecycle of medical consumables,and to explore application value of digital technique in improving efficiency of management for supply chain of hospital.Methods:A hierarchical technical architecture consisting of data layer,network layer,consensus layer,smart contract layer and application layer was adopted.Three mainly functional modules,i.e.,intelligent procurement,dynamic inventory management and full process traceability,were configured to construct the integrated intelligent supply chain system for full life cycle of medical consumables.Data were collected from 3 comprehensive third-level grade-A hospitals of Chongqing,including data of 150 072 surgeries and related data of medical consumables that were used in surgery from January 2023 to December 2024.Among them,the data of 74 316 surgeries from January to December 2023 were assigned as the pre-implementation group,and 75 756 surgeries from January to December 2024 were assigned as the post-implementation group.The average time of consumables requisition,days of inventory turnover,cost of consumable consumption per operation,error rate of billing,and consistency rate between accounts and materials of 3 hospitals between before and after the system was applied were compared,and the change of intervention effects after the system was applied also was compared and analyzed.Results:After this system was implemented,the average time of consumables requisition,days of inventory turnover,cost of consumable consumption per operation and average error rate of billing of combined effect of 3 hospitals were respectively(2.7±0.8)h,(6.8±1.5)d,(10 980.2±1210.7)CNY and(0.8±0.3)%,all of which were significantly lower than those before it was implemented,and the differences were significant(t=35.62,43.75,17.93,40.81,P<0.05).The average consistency rate between accounts and materials was(99.1±0.5)%after it was implemented,which was significantly higher than that before it was implemented(t=29.47,P<0.05).The differences of the decreased degree of the average time of consumables requisition,average days of inventory turnover,average cost of consumable consumption per operation,average error rate of billing,and the increased degree of consistency rate between accounts and materials of intervention effect among 3 hospitals were significant(F=9.05,10.12,13.08,8.27,29.75,P<0.05),which had higher heterogeneity(I 2>50%).Conclusion:The application of the integrated intelligent supply chain system for full lifecycle of medical consumables can improve the efficiency of requisitioning medical consumables,and accelerate the inventory turnover of medical consumable,and control the cost of surgical consumables,and enhance the accuracy of billing consumable,and the level of consistency between account and materials.
