基于慢性病轨迹框架护理干预对糖尿病肾病患者疾病管理能力和生命质量的影响OA

Objective To explore the impact of nursing intervention based on the chronic disease trajectory framework on the disease management ability and quality of life of patients with diabetic nephropathy.Methods A total of 70 patients with diabetic nephropathy admitted to Gao'an People's Hospital from December 2022 to December 2024 were selected and divided into a control group and an observation group by random number table method,with 35 cases in each group.The control group was given routine care,while the observation group received nursing intervention based on the chronic disease trajectory framework.The changes in the scores of the Self-Management Behavior Scale for Diabetic Nephropathy,the Quality of Life Scale for Chronic Disease Patients(QLICD-GM),and the occurrence of complications before and after the intervention were compared between the two groups of patients.Results After the intervention,the scores of diet management,exercise management,medication management,blood glucose monitoring and complication prevention in the observation group were all higher than those in the control group(P＜0.05).After the intervention,the QLICD-GM score of the observation group was higher than that of the control group(P＜0.05).The incidence of complications in the observation group was lower than that in the control group(P＜0.05).Conclusion In the nursing of diabetic nephropathy,implementing nursing intervention based on the chronic disease trajectory framework can effectively enhance patients'self-management ability of the disease,reduce the incidence of complications,and improve the quality of life.
