目的 分析肺泡表面活性物质相关蛋白B(SP-B)基因多态性对血必净注射液联合人免疫球蛋白治疗病毒性肺炎的疗效影响.方法 纳入2022年1月至2023年12月予血必净注射液联合人免疫球蛋白治疗的病毒性肺炎患者160例.治疗7 d后采集患者外周静脉血,采用直接测序法进行基因测序,分析不同SP-B基因型与疗效、炎症指标和肺功能的关系.结果 治疗7 d后,血必净注射液联合人免疫球蛋白对SP-B基因-18位点C/C基因型患者的疗效优于A/A+A/C基因型患者(P<0.05),超敏C反应蛋白(hs-CRP)、肿瘤坏死因子-α(TNF-α)和白细胞介素-6(IL-6)水平低于A/A+A/C基因型患者(P<0.05),用力肺活量(FVC)、用力呼出50%肺活量的呼气流量(FEF50)、最大呼气中期流速(MMFR)和氧合指数(OI)高于A/A+A/C基因型患者(P<0.05);SP-B基因1580位点T/T患者的疗效优于C/C+C/T基因型患者(P<0.05),hs-CRP和IL-6水平低于C/C+C/T基因型患者(P<0.05),FVC、FEF50、MMFR和OI高于C/C+C/T基因型患者(P<0.05).结论 血必净注射液联合人免疫球蛋白对SP-B基因-18位点C/C基因型病毒性肺炎患者的疗效、抗炎效果和肺功能康复效果优于A/A+A/C基因型患者,对SP-B基因1580位点T/T基因型患者的疗效、抗炎效果和肺功能康复效果优于C/C+C/T基因型患者.

AIM To analyze the effect of gene polymorphisms of pulmonary surfactant-associated protein B(SP-B)on the efficacy of Xuebijing injection combined with human immunoglobulin in treating viral pneumonia.METHODS From January 2022 to December 2023,160 patients with viral pneumonia who were treated with Xuebijing injection combined with human immunoglobulin in the hospital were selected as the study objects.Peripheral venous blood was collected after 7-day treatment,and gene sequencing was performed using the direct sequencing method to analyze the relationship between different genotypes of SP-B gene(SFTPB)and the efficacy,as well as inflammatory indicators and lung function.RESULTS After 7-day treatment,the efficacy of patients with C/C genotype at genomic position 18 in SFTPB was significantly higher than that of patients with A/A or A/C genotype(P<0.05).Levels of hypersensitive C-reactive protein(hs-CRP),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6)of patients with C/C genotype at genomic position 18 in SFTPB were lower than those of patients with A/A or A/C genotype(P<0.05).Levels of forced vital capacity(FVC),forced expiratory flow at 50%of forced vital capacity exhaled(FEF50),maximal mid-expiratory flow rate(MMFR)and oxygenation index(OI)of patients with C/C genotype at genomic position 18 in SFTPB were higher than those of patients with A/A or A/C genotype(P<0.05).The efficacy of patients with T/T genotype at genomic position 1580 in SFTPB was higher than that of patients with the C/C or C/T genotypes(P<0.05).Levels of hs-CRP and IL-6 of patients with T/T genotype at genomic position 1580 in SFTPB were lower than those of patients with the C/C or C/T genotypes(P<0.05).Levels of FVC,FEF50,MMFR,and OI of patients with the T/T genotype at genomic position 1580 in SFTPB were higher than those of patients with the C/C or C/T genotypes(P<0.05).CONCLUSION The efficacy,anti-inflammatory effect and pulmonary function rehabilitation effect of Xuebijing injection combined with human immunoglobulin in patients carrying C/C genotype at genomic position 18 in SFTPB with viral pneumonia are better than those in patients with A/A or A/C genotype.The efficacy,anti-inflammatory effect and pulmonary function rehabilitation effect of patients with T/T genotype at genomic position 1580 in SFTPB are better than those of patients with C/C or C/T genotype.
